Background and AimBy prospective studying the patiens who suffer from osteoporotic vertebral fractures(OVF)and treated by Hydraulic delivery vertebroplasty(HDVP),To compare the clinical efficacy and complications of HDVP by unilateral or bipedicular approach for OVF,thereby guiding the OVF treatment.Experimental methods and results Method:From June 2014 to June 2018,85 patients with osteoporotic vertebral compression fracture were randomly divided into two groups,44 patients(Group A)were performed by HDVP through unipedicular approach and 41 patients(Group B)through bipedicular approach.The patients were followed up for one year.Visual analogue scale(VAS)and Oswestry Disability Index(ODI)were used to evaluate the improvement of symptom and function.Height restoration rates and diffusion evaluation were used to analyze the vertebral reconstruction.Complication rates including cement extravasation and refracture were recorded and assessed to compare the safety and prognosis between groups.Result:In both groups,VAS and ODI improved significantly and remained relatively stable and there was no significant difference between the two groups at each follow-up time point.Mean high restoration rate of Group A and B were(25.67±2.78)% and(28.43±3.53)% at postoperatively(P>0.05).There were 9 and 10 asymptomatic cement extravasations and in Group A and B,respectively,with no statistical difference(P> 0.05).The operation time of group A was shorter than group B(P 0.05).Group B had more bone cement(P 0.05).The diffusion effect of bone cement in group B was better than that in group A.Conclusion: Both unipedicular and bipedicular HDVP are effective for the treatment of OVCF,but the former can shorten operation time,reduce X-ray exposure time,decrease damage,and reduce spending.
