Therapeutic Effect Of Two Treatment Methods On Open Ruedi-Allogwer Ⅲ Pilon Fracture

Objective:Therapeutic effect of two treatment methods on open Ruedi-Allogwer Ⅲ Pilon fracture Methods:The clinical data of 62 patients with open Ruedi-Allogwer Ⅲ Pilon fracture treated with ORIF in different time periods and in different ways were compared and analyzed.They were divided into two groups according to different operation methods: group A:emergency operation within 6 hours after injury,limited open reduction and internal fixation combined with external fixator treatment after thorough debridement of the wound,a total of 31 cases;group B: first give and debridement after injury.ORIF was performed on 7-15 days after the soft tissue recovered well.The surgical methods included 1/3 tubular plate fixation of fibula after reduction and distal tibia and inferior tibiofibular syndesmosis after reduction and bone grafting.A total of 31 cases were treated by suture,detumescence,infection prevention,calcaneal traction or plaster external fixation.The quality of fracture reduction,incidence of wound complications,fracture healing time,Burwell-Charnley score and Mazur standard ankle function score were compared between the two groups.Results:There were no significant differences in general data,fracture reduction quality,fracture healing time,Burwell-Charnley score,Mazur ankle joint function score and postoperative complications between the two groups.Group B was better than Group A in fracture healing time,and the difference was statistically significant.Conclusion:For open Ruedi-Allogwer Ⅲ Pilon fracture,emergency operation within 6 hours after injury and second-stage debridement are the better treatment methods.Their clinical effects are similar,but emergency limited open reduction and internal fixation combined with external fixator can significantly shorten the average healing time of fracture.Therefore,reasonable treatment should be selected according to the specific conditions of patients.
