Objective: To compare the Clinical efficacy of robot-assisted TKA and traditional TKA,and to determine whether robot-assisted TKA can provide better prosthetic lower limb mechanical axis alignment and clinical prognosis..Methods: Clinical controlled study focus on robot-assisted versus traditional TKA were retrieved from Embase,Pubmed,Web of Science,CNKI,Wanfang, and VIP Database to collect related articles published before November 2018.The articles were screened according to inclusion and exclusion criteria. After quality assessment and data extraction, statistical software review manager 5.3 was used for data-analysis.Results: Six studies with 253 cases for robot-assisted TKA and 231 cases for traditional TKA were included in meta-analysis.Through comparison,robot-assisted TKA was significantly superior to the traditional TKA group in terms of the lower limb mechanical axis alignment angle[WMD=-1.00,95%CI(-1.66,-0.35),P=0.003] and deviation of greater than 3° from neutral in mechanical axis rate[RR=0.04,95%CI(0.01,0.13),P=0.004],but the operation time[WMD=0.06,95%CI(-5.43,5.55),P=0.98] was longer than the traditional group;and we found no statistical difference between two groups on tibial angle, femoral angle, range of knee joint motion and complication. Conclusion : The use of robotic assisted systems in TKA surgery can significantly improve the accuracy of prosthesis fixation, achieve better reconstruction of lower limb mechanical axis alignment, and has the potential to reduce postoperative pain and promote recovery of knee function.
