Objective Sepsis is frequently encountered and highly lethal conditions among children treated in PICU;Also children with sepsis had a high incidence of malnutrition.The author investigate the nutritional status of children with sepsis in PICU and analyze the relationship between the nutritional status and prognosis of children with sepsis;Methods For sepsis /severe sepsis/septic shock patients admitted to the pediatric intensive care unit(PICU)during the year 2015 July to 2016 March,the patient were divided into sepsis group and severe sepsis / septic shock group,non-underlying disease group and ubderlying disease group;Height and weight was examined within 24 h after admission,Using WHO Anthro and Anthro Plus WFA and BMI/A software to calculate the Z value in order to determine the nutritional status of children.And compare each group malnutrition prevalence rate and mortality rate,in average intake,the average intake rate,mechanical ventilation time,PICU stay,the PCIS scores.Result 1.A total of 52 patients with sepsis group 19 cases,the rate of malnutrition was 36.8%,the mortality rate was 21.1%,in the actual average daily energy intake was 1 8 9.5 ±1 8.8 kj/(kg·d),the average energy intake rate was 60.3±1 0.3 %,the duration of mechanical ventilation was 5.2± 1.9d;2.Severe sepsis and septic shock group 33 cases,the rate of malnutrition was 39.4%,the mortality rate was 27.3%%,in the actual average daily energy intake was 1 6 6.2 ±1 7.4 kj/(kg·d),the average energy intake rate was 53.9±8.7 %,the actual average daily protein intake was 1.35±0.28 g,mechanical ventilation time was 9.4±2.4d;3.Between the sepsis group and severe sepsis / septic shock group of malnutrition prevalence rate,mortality rate,the actual average daily energy intake,the average energy intake rate,mechanical ventilation time were statistically significant difference(P<0.05);and severe sepsis group compared with the sepsis group,the average daily intake of protein,average protein intake rate were high,PICU longer hospitalization,but had no statistical difference(P>0.05).4.Underlying disease group of 36 cases,the rate of malnutrition was 30.6%,the mortality rate was 22.2%,non-underlying disease group of 16 cases,the rate of malnutrition was 56.3%,the mortality rate was 31.3%,with non-underlying disease group and ubderlying disease group two groups of malnutrition prevalence rate and mortality rate were significantly different was P<0.05,5.Underlying disease group has a higher actual daily average energy / energy / protein intake,protein intake and average intake rate than the non-underlying disease group,also the mechanical ventilation time and PICU stay were longer,but there was no significant difference(P>0.05).Conclusion 1.Children with sepsis have a high prevalence and mortality rate of malnutrition;2.Nutritional intake,malnutrition prevalence,mechanical ventilation time,PICU length of stay were associated with the severity of the disease and the underlying diseases in the patients with sepsis;3.The mechanical ventilation time and PICU length of stay were longer in the patients with less energy and protein intake. |
