Objective : Screening for colorectal cancer in the outpatients of the Fourth Hospital of Changsha and analyzing the causes of colonoscopy compliance to providing a reference for future screening strategies for colorectal cancer.Methods : Screening for colorectal cancer was performed on patients aged 40-75 who were admitted to the outpatient clinic of the Fourth Hospital of Changsha from January 1,2018 to December 31,2018.Risk assessment and FOBT screening method in the initial screening stage,for patients with positive screening,they are advised to perform a colonoscopy fine screening.Summarize the results of the primary screening and fine screening for further analysis,and analyze the reasons that affect the compliance of colonoscopy.At the same time,analyze the reasons for the influence of colonoscopy compliance and rejection of colonoscopy.Results: 1.A total of 3248 people participated in the risk assessment of colorectal cancer,among which 427 were positive in risk assessment,and the positive rate was 13.1%.A total of 382 people had FOBT,and 82 people with FOBT positive,the positive rate was 21.5%.A total of 433 people were screened positive,and the initial screening rate was13.3%.There was no significant difference in the positive rate of risk assessment(P=0.326),FOBT positive rate(P=0.901)and initial screening positive rate(P=0.319).2.A total of 251 people participated in fine screening,and the compliance of colonoscopy was 58.0%.A total of 38 target diseases were detected,and the positive rate of fine screening was 15.1%.The older the age,the higher the positive rate of fine screening(P=0.008);the difference of the positive screening rate of different genders was not statistically significant(P=0.554).3.The reasons for the compliance of colonoscopy were marital status,colonoscopy and positive screening results(P<0.001).Consensus colonoscopy as the dependent variable,the above reasons were independent variables,logistic regression analysis showed that married(P=0.001,OR=20.932),colonoscopy(P<0.001,OR=9.201),initial Screening results for colorectal cancer risk assessment and FOBT were positive(P < 0.001,OR = 41.849)is the cause of colonoscopy.4.The reasons for rejecting colonoscopy are “under-recognition of the disease”,“considering pain or trouble with colonoscopy” and“economic reasons”.Conclusion:1.Opportunistic screening is of great significance for the detection of colorectal cancer and its precancerous lesions.2.There was no significant difference in the positive rate of riskassessment,FOBT positive rate and initial screening rate of colorectal cancer in different genders.3.The older the age,the higher the positive rate of fine screening;the difference of the positive screening rate of different genders was not statistically significant.4.Married,colonoscopy,and primary screening results for colorectal cancer risk assessment and fecal occult blood test are positive for colonoscopy.5.The reasons for rejecting colonoscopy are “under-recognition of the disease”,“considering pain or trouble with colonoscopy”,and“economic reasons”.
