Objective: Intracerebral hemorrhage(ICH)is the second most common type of stroke.Because of its high incidence,high disability rate and high mortality rate,ICH seriously threatens human health,and 50%-70% of patients have a history of hypertension.Studies have shown that CD47,an integrin-related protein,plays an important role in ICH.Our aim was to investigate the relationship between CD47 and the degree of brain edema,neurological impairment and clinical prognosis in patients with hypertensive intracerebral hemorrhage(HICH).Methods: From September 2016 to April 2018,we collected the clinical data of 12 patients with HICH who were hospitalized in the Neurological Care Unit of Linyi People's Hospital,and all patients underwent improved-stereotactic soft-channel minimally invasive surgery of intracranial hematoma.The hematoma fluid and peripheral blood before the operation were taken to measure the volume of cerebral hemorrhage and edema during the operation.Meanwhile,Glasgow coma scale(GCS)and National Institute of Health stroke scale(NIHSS)were also evaluated.Modified Rankin Scale(mRS)and NIHSS were assessed 3 months after the onset of HICH.In Linyi People's Hospital,12 healthy people were collected as control group,and their fasting peripheral blood was taken.All blood samples were stored in a refrigerator at-80? for examination,and then the concentration of CD47 in hematoma,peripheral blood and peripheral blood of HICH patients and controls were detected by ELISA.The average concentration of CD47 in hematoma fluid of HICH group was calculated,and HICH patients were divided into high concentration group and low concentration group.The basic information between HICH group and control group,and the basic information of CD47 high concentration group and low concentration group were compared.Chi-square test was used to compare sex,t test was used to compare systolic blood pressure,diastolic blood pressure,white blood cell count,blood sugar,edema volume,GCS score,NIHSS score and mRS score.One-was ANOVA was used to compare the CD47 concentration in peripheral blood and hematoma fluid of HICH group and in peripheral blood control group.To analyze the correlation between the expression of CD47 and the degree of brain edema,neurological deficits and clinical prognosis in HICH patients.Results: 1.Systolic and diastolic blood pressures in HICH group were significantly higher than those in healthy control group,and the difference was statistically significant(P < 0.05),which met the criteria of hypertensive intracerebral hemorrhage;white blood cells and blood sugar in HICH group were significantly higher than those in healthy control group,and the difference was statistically significant(P < 0.05);there was no difference between the two groups of gender and age(P > 0.05).There was no significant difference in sex,age,systolic blood pressure,diastolic blood pressure,white blood cell count,blood sugar,onset time,bleeding volume between CD47 high concentration group and low concentration group(P > 0.05).2.The concentration of CD47 in hematoma fluid of HICH patients was significantly higher than that of patients' peripheral blood and healthy control group(P < 0.0001).3.Compared with the low concentration group,the edema around HICH focus was more serious,the GCS score was lower and the NIHSS score was higher in the high concentration group(P < 0.05).4.Compared with the low concentration group,the high concentration group of CD47 had significantly higher mRS score and higher NIHSS score 3 months after onset(P < 0.05).Conclusion: The expression of CD47 in the hematoma fluid increased significantly after HICH,and the higher the concentration of CD47,the larger the volume of brain edema,the more serious the neurological impairment,and the more serious the neurological impairment after 3 months.That is,the higher the expression of CD47 indicates the worse condition and prognosis.
