Objective 1.Through the analysis of the data of serum uric acid level and Hamilton Depression Scale(HAMD)in the acute phase of depression and bipolar affective disorder depressive episode,the changes of serum uric acid level and HAMD score in the acute stage and after treatment of depression and bipolar affective disorder were discussed.2.Comparing the difference of serum uric acid level between depression and bipolar affective disorder depressive episode in acute stage,to knowledge whether serum uric acid level can be used as a biological index to distinguish depression from.Methods Ninety-four patients with acute depression hospitalized in Yantai Psychological Rehabilitation Hospital were included in the study as depression group(34 males and 60 females).Thirty-nine patients with bipolar depressive episode in Yantai Psychological Rehabilitation Hospital were treated as bipolar group(16 males and 23 females).Eighty-one healthy people who were examined in Yuhuangding Hospital of Yantai were taken as control group(35 males and 46 females).All subjects were fed a ordinary diet.Fasting blood was taken from the control group on the same day,and fasting blood was taken from the depression group and the bipolar group within 24 hours after admission and 4 weeks after treatment,respectively,to test the serum uric acid level.The severity of symptoms was assessed by Hamilton Depression Scale(HAMD-17)at baseline and 4 weeks after treatment in depression group and bipolar group,respectively.Depression group and bipolar group were divided into mild-to-moderate subgroup and severe subgroup according to baseline HAMD score.Baseline blood uric acid levels were compared between the depressed,biphasic,and control groups.The changes of serum uric acid level in the depression group and the biphasic group were compared before and after treatment for 4 weeks.The changes of HAMD scores before and after treatment in the depression group and the biphasic group were compared.There was no difference in baseline blood uric acid levels between the mild to moderate subgroup and the severe subgroup in the acute phase of depression.There was no difference in baseline blood uric acid levels between the mild to moderate subgroup and the severe subgroup in the acutephase of depressive episodes of bipolar disorder.All data were statistically analyzed using SPSS 23.0.The Kruskal-Wallis H test was used to compare the age and gender composition of the three groups.The Kruskal-Wallis H test was used to compare the blood uric acid levels between the three groups.The comparison between the disease course and the baseline uric acid level between the depression group and the biphasic group was used.Mann-Whitney U rank sum test;Wilcoxon signed rank test was used to compare uric acid and HAMD scores before and after treatment in depression group and biphasic group;Mann-Whitney U was used to compare blood uric acid levels in mild and severe subgroups of depression group and biphasic group Rank sum test.The difference was statistically significant at P < 0.05(bilateral).Results There was no significant difference in age,gender composition and baseline BMI between the depression group,the bipolar group and the control group(P > 0.05).There was no significant difference between the depression group and the bipolar group in terms of course of disease,baseline HAMD level.(P > 0.05).Baseline serum uric acid level in the depression group were lower than those in the bipolar group and the control group,with statistically significant differences(all P < 0.05),while there was no significant difference in baseline serum uric acid level between the two groups(P > 0.05).After 4 weeks of treatment,the level of serum uric acid in depression group was higher than that in baseline level,and the difference was statistically significant(P < 0.05);there was no statistical difference in the level of serum uric acid before and after treatment in bipolar group(P > 0.05);the HAMD scores in both groups were lower than that in baseline level after treatment,and the difference was statistically significant(P < 0.05).The baseline serum uric acid level in depression group was significantly higher than that in severe depression group(P < 0.05).There was no significant difference in baseline uric acid level between mild and moderate depression group and severe depression group(P > 0.05).Conclusions The level of serum uric acid may be used as a biological index to indicate the acute stage of unipolar depression;the severity of unipolar depression may be assessed by the level of serum uric acid;the level of serum uric acid may be used as a biological index to distinguish unipolar depression from bipolar depression,but further studies with perfect design and large sample size are needed.
