Objective To compare the characteristics of cognitive function of patients with bipolar disorder I with manic,bipolar disorder I with depression and major depression disorder,and analyze the related factors.Methods In this study,51 patients with bipolar disorder I with depression(BD-D group),42 patients with bipolar disorder I with manic(BD-M group),56 patients with major depression disorder(MDD group)were enrolled in the outpatient and inpatient departments of Wuhan Mental Health Center,and 53 healthy controls(H group)were recruited in our hospital or in the society,and the MATRICS consensus cognitive test was conducted,and general clinical data were collected,The cognitive function and related factors of each group were compared and analyzed.Results The cognitive function of BD-D group,BD-M group and MDD group was lower than that of H group(P<0.05);The information processing speed of BD-D group was lower than that of MDD group(P<0.05).The information processing speed and working memory score of BD-D group were negatively correlated with HAMA score;The scores of word learning and memory were positively correlated with the first depressive symptoms and the number of relapses,and negatively correlated with the scores of education level and HAMA;The score of reasoning and problem-solving was positively correlated with the number of recurrences and negatively correlated with the HAMA score.The score of information processing speed in BD-M group was positively correlated with positive family history and smoking history;The score of working memory was positively correlated with the score of BRMS;Word learning and memory scores were positively correlated with positive family history and smoking history;Visual memory and learning scores were positively correlated with drinking history.The score of information processing speed in MDD group was positively correlated with age and marriage,and negatively correlated with HAMA score;Word learning and memory scores were negatively correlated with HAMA and family history;Visual memory and learning scores were negatively correlated with HAMA scores;The scores of reasoning and problem solving are negatively correlated with the scores of education level and HAMA;Social cognition is positively correlated with age,HAMD score and marriage.Conclusion Patients with bipolar disorder I and major depression disorder have varying degrees of impairment in the six dimensions of MCCB cognitive function compared to healthy controls.Patients with bipolar disorder type I depression performed the worst in information processing speed,while patients with bipolar disorder I manic performed the worst in visual memory and learning.There is a correlation between different areas of cognitive function of patients with bipolar disorder I and major depression disorder and demography factors or clinical variables.Due to the widespread cognitive impairment during the onset of both diseases,early intervention is necessary in the diagnosis and treatment process,while considering the impact of adverse factors to delay or improve cognitive impairment,assist in disease recovery and prognosis development. |
