ObjectiveTo explore the level of anxiety and depression,self-care ability and quality of life of patients with ileostomy,and the occurrence of complications of ileostomy.In addition,the influence of nurse-physician collaboration model on the level of anxiety and depression,quality of life after discharge,self-care ability of patients with ileostomy and complications of ileostomy was also explored.MethodsSelecting 100 patients as experimemtal cases who were admitted to Huangdao branch of the affiliated hospital of qingdao university from December 2016 to June 2018.According to random number table,the patients were randomly divided into two groups:the experimental group(n=50)and the control group(n=50).The control group received routine nursing care,while the experimental group received nurse-physician collaboration model care as well as routine nursing.Nurse-physician collaboration model care includes psychological support,education related to the ileostomy including self-care skills training,and the intervention time from 1 to 3 months after surgery.The frequency of measurement of both the self-care scale and quality of life scale for the two groups was 1day prior to surgery,1 month and 3 months postoperatively.Anxiety and depression scale was used to assess the level of the two groups on 1 day before surgery,1 day before discharge,1 month after surgery and 3 months after surgery.Results1.During the period of follow-up,1 patient in the control group died,1 patient was transferred to another hospital for treatment as a result of severe complications,1 patient was loss to follow up,so there are 47 effective cases in total.There was no death in the experimental group,but 1 patient needed to be transferred to another hospital for treatment due to the deterioration of the condition,1 patient was loss to follow up;therefore,there are 48 effective cases in the experimental group.The clinical data of the two groups is balanced and comparable.2.The four subentry scores on self care ability and self concept,self responsibility,self-care knowledge,and self-care skills are significantly higher in 1 month and 3 months postoperatively than that of before discharge in the experimental and control group respectively.Moreover,the scores in the experimental group are significantly higher than that in the control group,and the difference is statistically significant(P<0.05).3.There is no significant difference on quality of life scale in the functional areas of cognitive and social function of two dimensions in the experimental and the control group in 1 month after discharge(P>0.05).However,the score in 13 other dimensions is better in the experimental group than that of the control group(P<0.05).Furthermore,the score of15-dimension quality of life is higher in the experimental group than that of the control group in 3 months after discharge(P<0.05).4.SAS and SDS scores of patients in the experimental group are significantly lower than those of the control group on 1day before surgery,1day before discharge,1 month after surgery and 3 months after surgery,and the difference is statistically significant(P<0.05).5.The occurrence of skin complications and skin mucosa separation is significantly higher in the control group in 1 month after surgery compared to the experimental group(P<0.05).The patients in the control group had significantly higher risk of complications in the two aspects of skin complications and stenosis in 3 months after surgery(P<0.05).Conclusion1.Nurse-physician collaboration model care can significantly improve the patients' knowledge and skills of self-care,self concept and self responsibility.2.Nurse-physician collaboration model care can significantly improve the overall health and functional area score of patients,reduce the score of symptom area and other single items,and improve the quality of life of patients.3.Nurse-physician collaboration model care can significantly improve patients' negative emotions such as anxiety and depression,and improve patients' psychological conditions as well.4.Nurse-physician collaboration model care can significantly reduce the incidence of patients' ileostomy complications.
