ObjectivesTo establish health education model of gestational diabetes mellitus using the tea ch-back,and explore the influence of the teach-back model with gestational diabetes mellitus patients on blood glucose and health literature and pregnancy outcome.At th e same time,in order to reduce hyperglycemia and adverse pregnancy outcome and improve the quality of life,and provide clinical basis for transform health education methods,and prevent the occurrence and development of gestational diabete smellitus complications.MethodsThe study is quasi-experimental study,From December 2017 to July 2018,90 pa tients have been diagnosed of gestational diabetes mellitus by OGTT and have signe d the informed consent,gestational diabetes mellitus patients were divided into two different groups in the morning and afternoon at random,the experimental group have45 patients,while the control group have 45 patients.The intervention was implement ed from 24~28 weeks of pregnancy to leave hospital.The control group received rout ine health education,while the experimental group received health education model of the teach-back method,before 36 weeks,once two weeks,after 36 weeks,once a w eek,there are 40 minutes each time.two groups received health education for 8 tim es.The fasting blood glucose(FPG),blood glucose(2hPBG),glycosylated hemoglobin(HbA1c)and health literacy of the two groups were assessed before and after interv ention,pregnancy outcome indexes were assessed after intervention.Microsoft Excel 2003 was used for data double-check input,and IBM SPSS21.0 statistics software was used for data analysis,The independent-T test,chi-square test,and rank sum test of variance analysis were used to do the statistical analysis,and explore the influence o f the teach-back mode for patients with gestational diabetes mellitus on blood glucos e and health literature and pregnancy outcome.Results1.Situation of Sample loss.In this study,there are 90 patients with gestational diabetes mellitus were selected and randomized into an experimental group and a control group,with45 patients in each group.During the study,there are 2 patients can't check on time in the experimental group withdraw from research,and the control group 3 patients withdrew from the study due to insulin therapy and high blood pressure during pregnancy.there are 43 patients in the experimental group and 42 patients in the control group In the end.2.Comparison of general data between two groups of gestational diabetes patients.In the two groups,the patients with gestational diabetes mellitus were analyzed in terms of age and body mass index occupation and marital status and household incomes per capita and education level and combined with other pregnancy complications and gestational week and OGTT and so on,and there were no statistically significant(P>0.05).3.Effects of teach-back on blood glucose and health literacy and pregnancy outcome for patients with gestational diabetes mellitus.(1)Comparison of blood glucose control between two groups of gestational diab etes patients.Before the intervention,the fasting blood glucose,2h postprandial blood and glucoseand hemoglobinA1 c of the experimental group were no statistically signi ficant;After the intervention,the fasting blood glucose,2h postprandial blood glucose and hemoglobinA1 c of the experimental group were lower than the control group,an d the differences were statistically significant(P<0.05).(2)Comparison of health literature between two groups of gestational diabetes mellitus patients.After the intervention,The total scores of health literacy and the abili ty of communicate and the ability of application were higher than the control group,t hey were statistically significant(P<0.05);there was no significant difference in the a bility of application between the two group.(3)Comparison of pregnant woman between two groups of gestational diabetes mellitus patients.After the intervention,in the experimental group gestational hypertensi on and cesarean section rate were lower than the control group,there was statistically significant(P<0.05);after the intervention,there was no significant in the rate of infe ction and polyhydramnios between the two groups(P>0.05).(4)Comparison of neonatus between of two groups gestational diabetes mellitus patients.After the intervention,The birth rate of macrosomia in the experimental gro up was lower than the control group(P>0.05);There was no significant in neonatal as phyxia and preterm birth rate between the two groups(P>0.05).ConclusionsTeach-back is an effective health education mode more than traditional health ed ucation,it can control blood glucose level and enhance health literature and quality of life,and it also improve pregnancy outcome for patients with gestational diabetes mellitus,and it deserved to be clinical generalized value.
