Objectives To investigate the levels and clinical significance of plasma albumin,bilirubin and uric acid in patients with post-stroke depression(PSD).Methods A total of 230 patients with acute ischemic stroke who were hospitalized in the four wards of the Department of Neurology,Affiliated Hospital of North China University of Technology,from August 2017 to March 2018,were enrolled.General clinical data were collected and used in Hamilton.The Depression Scale(HAMD-24)was used to assess depression;according to the HAMD score,the PSD group(total score ≥ 8 points)and the non-PSD(NPSD)group(total score < 8 points),PSD based on the score The components were PSD mild group(8-20 points),PSD moderate group(20-35 points)and PSD severe group(>35 points);the levels of plasma ALB,TBIL and UA were measured and compared in each group;The patients were surveyed with a self-designed general situation questionnaire.The severity of stroke was assessed with the National Institution of Health Stroke Scale(NIHSS)and the activities of daily living(ADL)were assessed.Results 1 There were significant differences between PSD group and NPSD group in age,family history of mental illness and number of stroke(onset/recurrence)(P < 0.05).There were no significant differences in gender,marriage,education,body mass index(BMI),diabetes,hypertension,coronary heart disease,hyperlipidemia,smoking,drinking and other aspects(P > 0.05).2 The incidence of depression was 46.1% in patients with ischemic stroke,of which 51 cases(48.1%)were mild,29 cases(27.4%)were moderate and 26 cases(24.5%)were severe.3.The correlation analysis of NIHSS score with albumin,bilirubin and uric acid showed that NIHSS score was not correlated with albumin,bilirubin and uric acid(r=-0.053,-0.043,0.084,P > 0.05),that is,there was no correlation between the degree of neurological deficit and the levels of these three indicators.4 The levels of plasma bilirubin and albumin and uric acid in PSD group were higher than those in NPSD group(P < 0.05).5 The levels of serum albumin,bilirubin and uric acid in mild,moderate and severe groups of PSD had statistical significance(P < 0.05);after multiple comparisons,there was no significant difference in the levels of albumin between mild and moderate groups(P > 0.05),and the levels of albumin in mild and moderate groups were higher than those in severe groups(P < 0.05).There was no significant difference in bilirubin level between mild group and moderate group,moderate group and severe group(P > 0.05),but mild group was significantly smaller than severe group(P < 0.05).The level of uric acid in mild group and moderate group was lower than that in severe group(P < 0.05),and there was no significant difference between mild group and moderate group(P > 0.05).6.The NIHSS score of PSD group was higher than that of NPSD group,and the ADL score of PSD group was lower than that of NPSD group,the difference was statistically significant(P < 0.05);the ADL score of mild depression and severe depression group was higher than that of moderate depression group(P < 0.05),and there was no significant difference in NIHSS score among the three groups(P > 0.05).There was no significant difference between the two groups(P > 0.05).The score of the mild group was significantly higher than that of the severe group(P < 0.05).The correlation analysis showed that plasma albumin and uric acid levels were negatively correlated with HMAD scores(r=-0.277,-0.264,P < 0.05),and plasma bilirubin levels were positively correlated with HMAD scores(r=0.304,P < 0.05).Logistic regression analysis showed that age(OR = 1.034,95% CI 1.005-1.063,,P < 0.05),family history of mental illness(OR = 6.548,95% CI 1.283-33.422,P < 0.05),non-first-onset stroke(OR = 2.392,95% CI 1.322-4.322-4.329,P < 0.05)were probably risk factors for PSD.Albumin(OR = 0.967,95% CI 0.941-0.994,P < 0.05)and uric acid(OR = 0.989,95% CI 0.980-0.997,P < 0.05)were the probably protective factors of PSD.Conclusions 1 The levels of serum albumin,bilirubin and uric acid in patients with PSD were different from those in patients with NPSD,and there were differences in the levels of serum albumin,bilirubin and uric acid in patients with different levels of depression.2 Old age,family history of mental illness and non-first stroke were probably risk factors for PSD.Albumin and uric acid were probably protective factors for PSD.Figure0;Table9;Reference 144...
