Objective:Cerebral infarction occupies almost 70%of all the cerebral vascular diseases.To investigate the role they play in occurring and developing of cerebral infarction,our study focuses on the concentration of serum bilirubin and uric acid in patients with cerebral infarction.Method:233 patients with cerebral infarction were involved in this study.Those who were admitted within 3 days after the onset of the neurological deficit were divided into acute cerebral infarction group;those within 3-14 days were divided into sub-acute group.Those NIHSS≥5 were divided into moderate and severe group; NIHSS≤4 into mild group.All the patients were taken fasting blood sample to detect serum total bilirubin,direct bilirubin,indirect bilirubin,uric acid,Homocysteine(Hcy), Triglycerides(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C) the next morning of admission and have color Doppler ultrasonography of the carotid arteries within three days after admission.Control group includes 187 healthy employees of our hospital. They were taken fasting blood sample in the morning to detect serum total bilirubin, direct bilirubin,indirect bilirubin and uric acid.Result:Compare the patients with or without hypertension,coronary heart disease, diabetes,cerebral vascular disease history,the concentration of serum bilirubin doesn't have statistically significant difference.The concentration of serum total bilirubin and direct bilirubin in patients with cerebral infarction is statistically higher versus controls.Moderate and sever group has higher serum bilirubin concentration compare with mild group.A statistically significant rise in serum bilirubin was found in patients with acute cerebral infarction compare with sub-acute group and controls, but the sub-acute group doesn't have significant difference with controls.A statistically significant positive correlation was observed between serum bilirubin and HDL-C and Hcy in patients with cerebral infarction.The cerebral infarction patients with diabetes have higher serum uric acid concentration than those without diabetes.The concentration of serum uric acid in patients with cerebral infarction is statistically higher versus controls.Moderate and sever group has lower serum uric acid concentration compare with mild group.A statistically significant rise in serum uric acid was found in patients with acute and sub-acute cerebral infarction patients compare with controls,but no significant difference was found between acute group and sub-acute group.A statistically significant negative correlation was observed between serum uric acid and HDL-C in patients with cerebral infarction.Conclusion:1.Serum bilirubin and uric acid in patients of all OCSP types have no difference.All the TOAST types have no significant difference in serum bilirubin.Only SOE type has higher serum uric acid,the other four types have no significant difference.2.Serum bilirubin in patients with cerebral infarction is higher than controls.A statistically significant rise in serum bilirubin was found in patients with acute cerebral infarction compare with sub-acute group and controls,but the sub-acute group doesn't have significant difference with controls.Moderate and sever group has higher serum bilirubin concentration compare with mild group.All above indicate that serum bilirubin,as an anti-oxidant,elevates during acute period and decreases during sub-acute period.The more severe the neurological dysfunction is,the higher serum bilirubin appears.3.Serum uric acid in patients with cerebral infarction is higher than controls.A statistically significant rise in serum uric acid was found in patients with acute and sub-acute cerebral infarction compare with controls,but the acute group doesn't has significant difference with sub-acute group.Moderate and sever group has lower serum uric acid concentration compare with mild group.All above indicate that the increase of serum uric acid is no just at acute period after cerebral infarction.After acute period,the serum uric acid is still high due to atherosclerosis.The moderate and severe group has lower serum uric acid maybe because consumption of uric acid as anti-oxidant.4.The cerebral infarction patients with diabetes have higher serum uric acid concentration than those without diabetes.In patients with diabetes,the production of uric acid is higher and the excretion is lower.For those diabetes patients,we should pay attention to serum uric acid concentration after cerebral infarction.5.In patients with cerebral infarction,serum bilirubin concentration has positive correlation to Hcy concentration.6.A statistically significant positive correlation was observed between serum bilirubin concentration and HDL-C concentration,and negative correlation between serum uric acid concentration and HDL-C concentration.
