Quality Of Life And Related Factors In Patients With Post-stroke Depression

Abstract/Summary:
Objective Post-stroke depression(PSD)is one of the most common symptoms of stroke.It not only severely impairs the daily living ability of patients,but also increases suicide concept of stroke patients,which directly affects their neurological rehabilitation,leading to an increase in disability,mortality and recurrence rate of stroke.The aim of this study was to explore the quality of life of PSD and the its influencing factors of post-stroke depression,such as stroke status,personal condition and external support,in order to provide theoretical basis for the treatment and nursing of stroke patients with depression.MethodsA total of 110 patients with post-stroke depression in Baoding Hospital from January 2017 to June 2018 were selected as the study subjects.Demographic data and stroke severity were collected by self-made questionnaires.Hamilton Depression Rating Scale for Depression(HAMD)scales was used to evaluate depression status and Stroke Specialized Quality of Life Scale(Stroke sc).A quality of life(SS-QOL)was carried out quality of life survey,and simple coping style questionnaire and social support assessment was used to collect the relevant information of the subjects.All data are input by Epidata 3.1,and statistical analysis is carried out by SPSS 17.0 statistical software.Counting data are expressed by rate or composition ratio,and measuring data are expressed by mean and standard deviation.Variance analysis,t-test and Pearson correlation analysis were used to analyze the single factor,and multiple stepwise regression analysis was used to analyze the quality of life of post-stroke depression.Result1 General information of the subjects:110 patients with PSD were investigated,aged 38-92 years,with an average age of 64.4±14.3 years.Male patients accounted for61.8%.The highest educational level is junior high school and below.The proportion of single persons(including divorced or widowed)in marital status was 27.3%.Cerebral infarction accounted for 52.7%,and the first stroke accounted for 54.5%.Among the subjects,the popular depressive patients were in the acute stage of stroke,followed by the convalescent stage,and the most severe depressive patients were in the sequelae stage.2The average total score of quality of life in 110 patients with PSD was 154.67.3Univariate analysis of quality of life in patients with PSD:The overall quality of life and the scores of each factor in patients under 65 years old were higher than those in patients over 65 years old.Except for the difference in family roles and emotions,the other parts score of quality of life were significance between under and over 65 years old(P<0.05).The linguistic score of quality of life in female patients with PSD was higher than that in male patients(P=0.022).There was no significant difference in other aspects of quality of life between men and women.There were significant differences in visual acuity of PSD patients with different educational levels.The quality of life score of married patients with PSD was higher than that of single patients with PSD,and there were significant differences in language and social roles between the two groups.The scores of total quality of life,language,activity,self-care ability,thinking,functional vision of upper limbs and working factors of PSD patients were urban>rural>urban in order from high to low(P<0.05).There was significant difference among the three groups(P<0.05).The difference between the two groups was significant(P<0.05).The score of family role factor in SS-QOL of PSD patients was the lowest in the<2000 yuan/month group,followed by the<2000 yuan/month group,and the highest in the<5000 yuan/month and above group.There was significant difference among the three groups(P<0.001).The emotional factor score of SS-QOL in patients with PSD was the highest in 2000 yuan/month group,which was significantly different from the other two groups.The visual factor score of SS-QOL in patients with PSD was the lowest in the group of 5000 yuan/month or above,which was significantly different from that in the group of less than 2000 yuan per month of family income.Among the factors of quality of life of PSD patients from different sources of medical expenses,only the scores of emotion and personality were significantly different among groups.The scores of self-paid PSD patients were significantly lower than those with medical insurance.The scores of emotion,personality,social role and visual acuity of PSD patients with different severity of stroke were significantly different among groups,with mild patients scoring the highest and severe patients scoring the lowest.The total scores of quality of life and each factor in the first stroke patients were higher than those in the second or more stroke patients.Besides family roles and visual factors,there were significant differences in other factors.The total scores of quality of life and other factors in patients with PSD were the lowest in severe depression group.Except family role factors,the scores of other factors and total scores of quality of life were significantly different among the three groups(P<0.05).The total scores of coping styles of PSD patients were positively correlated with the total scores of quality of life and energy,language,activity,personality,self-care ability,thinking,upper limb function,vision and work factors,while the total scores of social support were positively correlated with the total scores of quality of life and other factors.4 Multivariate stepwise regression analysis of quality of life score in patients with PSD.Age,depression and coping style are the factors affecting the quality of life of patients with PSD.The multiple correlation coefficient(R)and determination coefficient(R~2)of the regression equation are0.716 and 0.513 respectively.ConclusionsThe socres of working aspect was the lowest compared with other aspect score of quality of life of PSD.Its influencing factors mainly include age,depression degree and coping style.Our finding suggested that giving positive psychological support to patients with PSD and stimulating positive coping styles can improve the quality of life of patients with PSD.Figure0;Table18;Reference...
