ObjectiveThis study evaluated the outcome of treatment according to the Lund concept combined with sodium aescinate in patients with intracranial hypertension caused by severe craniocerebral injury and spontaneous cerebral hemorrhage.Methods and PatientsA retrospective study of neurosurgical patients in Tongji Hospital was performed on 15 patients who participated in the study.Four patients with severe craniocerebral injury and 11 patients with spontaneous cerebral hemorrhage participated in the study.During the treatment,intracranial pressure,cerebral perfusion pressure,electrolytes,albumin,hemoglobin and other indicators were monitored.Patients were followed up 2 and 6 months after treatment,and were divided into survival group and death group according to the follow-up results.All patients arrived at the hospital for treatment within 24 hours after injury or cerebral hemorrhage,with an average age of 57.6 years(38-77 years)and a median Glasgow coma score of 6(4-8).The mean preoperative initial intracranial pressure of all patients was 27.7mmHg(21-36mmHg).ResultOf the 15 patients who participated in the study,13 survived at 6 months of follow-up,with a survival rate of 86%.The good prognosis rate was 73%(Glasgow Outcome Score 4 or 5).The compliance rate of each index: intracranial pressure:84.8%,cerebral perfusion pressure: 97.0%,blood oxygen saturation: 99.73%,body temperature: 84.98%,blood glucose: 86.73%,blood sodium: 92.5%,serum potassium:94.17%,albumin: 89.84%,hemoglobin: 83.21%.ConclusionTreating patients with intracranial hypertension caused by severe craniocerebral injury and cerebral hemorrhage,according to the Lund concept combined withsodium aescinate,results in a favorable outcome when the protocol is followed.
