Objective:To understand the characteristics of medical disputes in public hospitals under the special system of Xinjiang Production and Construction Corps,analyze the causes of both doctors and patients leading to medical disputes,and identify the influencing factors affecting the rate of compensation for medical disputes.The research results provide reference for resolving medical disputes,improving medical quality,improving the relationship between doctors and patients,and formulating prevention and control measures for medical disputes.Methods:A cluster sampling method was adopted to select 349 valid medical disputes from 2002 to 2017 held by a third-level general hospital of the Corps,and the information on medical disputes was collected.Descriptive analysis method is used to analyze the overall situation of medical disputes such as the number of medical disputes,the incidence rate,the distribution of departments,the causes,the solutions and the professional identification.The descriptive analysis of the characteristics of the doctors and patients,and finally the medical disputes The factors affecting the compensation rate are analyzed.Results:1.From 2002 to 2017,the hospital has an average of 22 medical disputes per year,with an average annual compensation of 687,400 yuan;the medical disputes in the past decade have shown an overall downward trend,reaching the lowest level in 2017,every 100 medical personnel,each The number of medical disputes per 10,000 cases and the number of medical disputes per 10,000 cases were 0.65,2.40,and 0.12,respectively;51.86% of medical disputes occurred in the surgical department,and the top three in the Department of Orthopaedics,Cardiothoracic Surgery,and Emergency Department ranked the top three departments.84.8% of medical disputes have faults in medical treatment,49.86% of medical disputes generate economic compensation;the first three categories of medical disputes for medical reasons are treatment problems,medical ethics,doctor-patient communication problems,and the first three categories The lack of responsibility is negligence,surgical operation errors,and complications;the solution is based on consultation between doctors and patients(83.95%)and the compensation rate is the lowest;the overall responsibility rate for medical disputes is 81.13%,and medical accidents.Compared with the identification,the responsibility rate of judicial fault identification is relatively high at 93.94%,and the main responsibility and secondary responsibility account for more than 60%.2.Group characteristics of doctors and patients: Among the 349 medical disputes,68.77% of the medical staff were male,61.60% of the medical staff were intermediate and below,83.09% were clinicians;95.7% of the patients were Han,age 37.54% of those older than 60 years old,70.78% of junior high school and below,62.18% of patients were surgical patients,and the top three occupational groups were retired employees,employees of enterprises and institutions,and no fixed occupations.It reached 70.49%.The payment methods are mainly concentrated on urban workers' medical insurance and full self-pay,which are 161 and 114 respectively,which together account for nearly 80%.3.whether the transfer,department category,professional category,solution,professional identification and claim amount are the influencing factors of medical dispute compensation rate.4.nursing professional,medical technology department,mediation route,claim amount > 10,000 yuan is an independent risk factor for medical dispute compensation rate.Conclusion:1.The incidence rate of medical disputes in the sample hospitals from 2007 to 2017 showed an overall downward trend.The number of medical disputes and the amount of compensation were lower than the national average.However,it could not explain that the sample hospitals had high medical technology and may have a small population.It is related to social factors such as backward economic development,weak awareness of patient rights protection,and stability and high pressure.2.The lack of responsibility and surgical operation errors of medical staff are 24.32% and 18.24% respectively,which is the most important cause of medical disputes.The fundamental reason for alleviating medical disputes is to improve the medical technology level and service quality.3.The group of disputed patients showed the characteristics of Han nationality,the elderly,junior high school and below,the degree of education,retired workers,and medical insurance for urban workers.It was consistent with the background characteristics of the Corps population and showed the characteristics of the Corps.4.The medical staff group is mainly male,intermediate and below,and clinical major.5.The doctor-patient consultation channel compensation rate(43.69%)and the average compensation amount are lower than the main way for hospitals to resolve medical disputes before 2017.In 2018,the “three mediation and one insurance” model of medical disputes has been established and the people's mediation will become Main mediation channels for future medical disputes.;6.Nursing profession,medical technology department,mediation route,claim amount > 10,000 yuan and other factors exist when there is a greater possibility of compensation for medical disputes.At this time,the hospital medical dispute handling department should actively intervene to minimize the loss.
