Objective: To analyze the clinical and laboratory examinations,imaging findings and follow-up prognosis of children with congenital tuberculosis,and to strengthen the understanding of congenital tuberculosis to reduce misdiagnosis,mistreatment and prognosis.Methods: A retrospective analysis of 22 children diagnosed with congenital tuberculosis at the Children's Hospital of Chongqing Medical University from January 1,2000 to December 31,2018,and summarized the clinical and laboratory examinations,imaging characteristics,Follow-up prognosis.Results: The age of onset was 1 to 29 days,and the average age of onset was 15.7 days.All mothers had a history of tuberculosis,and most of them had tuberculosis.Clinical manifestations of fever and cough were the most common.Blood routines mostly showed white blood cells and CRP increased,the positive rate of ?-interferon release test was 62.5%,the positive rate of acid-fast bacilli smear was 36.8%,the positive rate of Mycobacterium tuberculosis PCR test,Mycobacterium tuberculosis culture was 40%,37.5%.The main manifestations of cerebrospinal fluid were protein elevation between 0.53-2.03g/L.Chest X-rays in 2 cases suggesting miliary nodules,chest CT in 2 cases as miliary nodules,and 11 cases with lymphadenopathy;There were 9 and 11 cases of hepatosplenomegaly and intra-organic lesions in color ultrasound and CT,respectively;14 cases were considered to have tuberculous meningitis,3 cases were lost to follow-up,6 cases died,and 1 case was drug-resistant tuberculosis,three patients were still under treatment,and 9 patients were better treated after treatment.Conclusion: Congenital tuberculosis is extremely atypical,difficult to detect early,and the mortality rate is high.History of tuberculosis,combined with laboratory testing Indicators,imaging findings,enhanced awareness and early detection of congenital tuberculosis,play an important role in controlling disease progression and improving prognosis,and reduce mortality.
