BackgroundIn recent years,with the increase of human autoimmune disease and population mobility,and abuse,irregular anti-tuberculosis drugs,made the incidence of Tuberculosis has a tendency to increase.At present,there is still lack on the basis research of spinal tuberculosis,such as the diagnosis,drug-resistant,as well as how to grasp the operation indication reasonably,and how to choice surgical approach,etc.there is still lack of methods of the diagnosis of spinal tuberculosis,particularly atypical spinal tuberculosis and early spinal tuberculosis.Clinically,Delays in the diagnosis of spinal tuberculosis is common.The diagnosis of the typical spinal tuberculosis is not difficult,but the diagnosis of the atypical spinal tuberculosis and the early spinal tuberculosis is still a big challenge to the clinical doctors.So,it is necessary to find a method,which could diagnose the spinal tuberculosis accurately and fleetly.ObjectiveTo summarize the imaging findings and laboratory tests' results of spinal tuberculosis,and to make a comparative analysis.Exploring how to diagnose spinal tuberculosis according to the existing examinations.Then we can offer help to the orthopaedic doctors to diagnose spinal tuberculosis.MethodsCollecting 65 patients by the Bone surgery of The first affiliated hospital of Xinxiang medical university(Tuberculosis hospital in Henan province),who were in the hospital in 2015.05-2016.06.They had been diagnosed of spinal tuberculosis by histopathology and anti-tuberculosis drugs treatment.All of them had been taken the X-ray examination,Computed Tomography(CT)and MRI examination.They also had been taken the tuberculin test(PPD),serum anti-tuberculosis antibody and T-SPOT.TB assay.Collect the basic clinical information,imaging data and laboratory test results of each patient,To compare the value of various detection methods in the diagnosis of spinal tuberculosis.ResultsWe had enrolled 65 patients with spinal tuberculosis,the average age of them were 40.7±10.6.There were 60 of them complained of back pain,accounted for 92.3%.There were 10 of them had mycobacterium tuberculosis poisoning symptoms,such as low grade fever,and night sweats,accounted for 15.4%.The patients had spent an average of 477.3 days from the symptoms appear to made a definite diagnosis this time.X-ray had a higher sensitivity compared with CT and MRI in the diagnosis of spinal deformity.The difference was significant statistically(P < 0.05).CT had a higher sensitivity compared with X-ray and MRI in the diagnosis of dead bone formation(P < 0.05 ?).MRI had a higher sensitivity compared with X-ray and CT in diagnosing vertebral abscess formation and the compression of epidural sac(P < 0.05).There had no significant differences between the three methods of imaging examination in diagnosing the Vertebral body clearance stenosis and intervertebral disc involvement.The positive rate of PPD(Scleroma diameter?15mm),serum anti-tuberculosis antibody and T-SPOT.TB respectively 46.2%,41.5%,and 86.2%.The positive rate of T-SPOT.TB was significantly higher than PPD skin test(P<0.05),and Serum anti-tuberculosis antibody(P<0.05).Conclusion(1)Three kinds of imaging findings had their own characteristics,but it had more help by the combination of all three.(2)T-SPOT.TB had higher sensitivity compared with traditional tuberculosis detection methods,which could be used as auxiliary diagnosis of spinal tuberculosis.
