Clinical Study Of Surgical Treatment Of Esophageal Foreign Body

Objective: To explore the effect and method of surgical treatment of esophageal foreign body.Methods: From December 2012 to January 2018,We retrospectively analyzed the clinical records of 21 patients undergoing surgical treatment for esophageal foreign bodies in our department.The clinical records include the general data(gender,age,clinical manifestations,type of foreign body,impaction site and time of foreign body,preoperative complications),operative records,postoperative complications and the results of treatment.Results: Among the 8 cases of cervical foreign body,6 cases chose the anterior incision of the left sternocleidomastoid muscle.Incisions were made at the anterior margin of the right sternocleidomastoid in 2 cases.Among the 13 cases of thoracic segment foreign bodies,1 case entered the chest through the left 6th intercostal space.4 cases went through the right fourth intercostal space;The right 5th intercostal space was selected in 7cases.The right 6th intercostal space was selected in 3 cases.Postoperative complications occurred in 8 cases(38.09%),including 2 cases ofesophageal fistula(9.52%),2 cases of right pyothorax(9.52%),and 6 cases of pulmonary infection(28.57%).6 cases(28.57%)of incision infection.All the 15 patients were cured and discharged from hospital,and the other 6patients were transferred to local hospitals for continuous intravenous nutrition support and acid inhibition treatment.All patients were followed up for 7 months to 5 years.Except that 1 patient died of esophageal cancer,the others had no dysphagia,food choking and other symptoms whose quality of life were not affected.Conclusion: According to esophageal anatomy,the location of incarcerated foreign body,preoperative complications,and the patient's own condition,the four factors jointly determine the surgical route and method.Surgical treatment of esophageal foreign body is effective,and after timely surgical intervention,postoperative complications and nutritional status of patients should also be paid attention to.
