Objective: To development and build a set of suitable for chronic disease classification management evaluation index system of chengdu, guided by the scientific and qualitative and quantitative research methods, guided by the principles of objective, fair, reasonable and efficient,so as to facilitate the site to pass the evaluation index system.Evaluate the grading management of chronic diseases in Chengdu,so as to understand the status quo of grading management of chronic diseases in Chengdu, further improve the quality and level of grading management of chronic diseases in Chengdu, and promote the sustainable development of grading management of chronic diseases. Methods: the evaluation index system was preliminarily drafted by using the method of literature research, expert discussion method, the final evaluation index system was determined by using the Delphi expert consultation method and the boundary value table method, the scientificity of the expert authority coefficient, positive coefficient and coordination coefficient were calculated by Using the relevant formula, the weights of each indicator in the indicator system was screened with the Delphi method,Ensure the rationality and stability of each indicator weight, and finally establish a quality evaluation index system. Results: Through the methods of literature and brainstorming by expert groups,4 first-level indicators,14 second-level indicators and 51 third-level indicators were initially established.Under the professional inspection of Delphi expert consultation, the authoritative coefficient was 0.837,the positive coefficient was 100%,and the coordination coefficient was 0.394.After the first round of expert consultation, the first-level indicators were more stable, and the second-level indicators were eliminated by one and the third-level indicators were eliminated by 16,After the second round of expert consultation, the final indicator system was determined. There were 4 first-level indicators,12 second-level indicators and 37 third-level indicators. The weights of each index were distributed between [0.0116,0.3217],and the overall weight of each indicator was more volatile and small.Discussion: This study focuses on the application of the Delphi method, the construction of the evaluation system, and the determination of the weight of the indicators. The system is representative and necessary, and can reflect the status quo better.Well, the implementation feasibility is strong, the expert coefficient of the index expert consultation is higher, the support for the research is greater, the opinions of the experts tend to be consistent, and the confirmation of each weight is more rational and less volatile. However, due to the low availability of information on some indicators,it is difficult to provide them in the actual situation. With the urgency of time and the limited resources,it is necessary to further explore the depth and comprehensiveness of some indicators.
