Orthostatic hypotension(OH)is a common cardiovascular disorder,with or without signs of underlying neurodegenerative disease.Its prevalence is affected by age and associated diseases and OH is one of the most common diseases in the elderly.OH is diagnosed on the basis of an orthostatic challenge.The magnitude of increase in systolic blood pressure in response to the shift from supine to upright posture is considered to reflect the adequacy of orthostatic regulation.Orthostatic integrity is largely maintained by the interaction between the skeletal muscle pump,neurovascular compensation,neurohumoral effects,and cerebral blood flow regulation.Epidemiological investigation and clinical study also found that,the occurrence of OH is related to mechanisms such as arteriosclerosis,autoimmune,genetic background,vitamin deficiency.Various physiological states and disease conditions may disrupt these mechanisms.OH can be an asymptomatic sign or manifest as symptoms that range from lightheadedness to loss of consciousness.OH may complicate treatment of hypertension,heart failure,and coronary heart disease;cause disabling symptoms,faints,and traumaticinjuries;and substantially reduce quality of life.Despite being largely asymptomatic or with minimal symptoms,the presence of OH independently increases mortality and the incidence of myocardial infarction,stroke,heart failure,and atrial fibrillation.OH and decreased cerebral blood flow are strongly related.Even OH has been associated to different degrees with impaired cognitive function,increased hopelessness,and decreased sleep quality.At present,clinicians should pay more attention to the etiology,pathogenesis,clinical impact,screening and treatment of OH.Therefore,an indepth understanding of OH can guide clinical practice and help us improve the life quality of patients.
