At present,the aging of the population has become an extremely serious social problem in China,which seriously affects the development of our society,economy and so on.With the increase of age,the immune function of the elderly group will gradually decline and the physiological function begins to decrease.Senile dementia,diabetes,cardiovascular and cerebrovascular diseases become common diseases in the elderly population.Among them,diabetes mellitus is a chronic metabolic disease,which can cause irreversible complications in the heart,lower limbs,kidneys and brain and atherosclerosis is one of the common complications.Especially carotid atherosclerosis,can lead to ischemic stroke moderate serious consequences.According to relevant research reports have confirmed that stable carotid plaque is relatively strong and not easy to rupture lead to ischemic stroke,and unstable plaque.However,unstable carotid plaque,like a ticking time bomb,can cause varying degrees of cerebral infarction,even if it does not cause severe vascular stenosis.So understanding the stability of plaque plays an important role in the prevention of ischemic stroke.Although the factors of influencing the stability of carotid plaque are complicated,this paper mainly studies the correlation between blood glucose status and carotid plaque stability in elderly type 2 diabetes mellitus,so as to provide guidance for the control of clinical blood glucose and the prevention of ischemic stroke.Purpose: To study the relationship between blood glucose status and carotid plaque stability in elderly patients with type 2 diabetes mellitus.To investigate the effect of fasting blood glucose and 2-hour postprandial blood glucose on carotid atherosclerosis in elderly patients with type 2 diabetes mellitus.Methods: A total of 100 elderly patients with type 2 diabetes mellitus who met the international diagnostic criteria were selected from the neurosurgery department of Chengde Central Hospital.The past and current medical history of the patients were collected,and then venous blood samples were collected to check fasting blood glucose and 2-hour postprandial blood glucose,serum total cholesterol,triacylglycerol.Then the carotid artery color Doppler ultrasonography was used to compare the blood glucose status among different stable carotid plaques and analyze the relationship between blood glucose and the stability of carotid plaques.Results: 1.Among the elderly patients with type 2 diabetes,there were statistically significant differences in age,triglycerides and cholesterol between the patients without carotid artery plaques and those with stable plaques and unstable plaques(p<0.05),total cholesterol,triglyceride and age in unstable plaque group were higher than those in the other two groups.And there were significant differences in the proportion of smokers and those with hypertension(p<0.05),the proportion of smoking and hypertension in unstable plaque group was higher than that in the other two groups.2.There was significant difference in fasting blood glucose level and 2-hour postprandial blood sugar level between patients with plaque and those without plaque(p < 0.05),the fasting and postprandial blood glucose of the carotid plaque group was higher than that of the non-carotid plaque group.There were significant differences in fasting blood glucose level and 2-hour postprandial blood sugar level between stable plaque group and unstable plaque group(P < 0.05),the blood glucose of the unstable group was higher than that of the stable group.3.For patients over 75 years old,there was no statistically significant difference in fasting blood glucose and 2-hour postprandial blood glucose between patients without plaque and those with plaque(P>0.05).There was no statistically significant difference in fasting blood glucose and 2-hour postprandial blood glucose between the stable plaque group and the unstable plaque group(P>0.05).Patients younger than 75 years old showed statistically significant differences in fasting blood glucose level and 2-hour postprandial blood glucose between patients in the non-plaque group and those in the plaque group(P<0.05),the fasting and postprandial blood glucose of the carotid plaque group was higher than that of the non-carotid plaque group.There were significant differences in fasting blood glucose and 2-hour postprandial blood glucose between stable plaque group and unstable plaque group(P < 0.05),the fasting and postprandial blood glucose of the unstable group were higher than that of the stable group.Conclusions: 1.Smoking history,age,history of hypertension and blood lipid all play an important role in the formation and development of carotid plaque in elderly patients with type 2 diabetes mellitus.2.The development of carotid atherosclerotic plaques in elderly patients with type 2 diabetes is correlated with fasting blood glucose and 2-hour postprandial blood glucose.Moreover,the stability and instability of plaque were correlated with fasting blood glucose and 2-hour postprandial blood glucose.The higher fasting blood glucose and 2-hour postprandial blood glucose,the more plaque tended to be unstable.3.In the age group older than 75 years old,fasting blood glucose and 2-hour postprandial blood glucose have little effect on the stability of plaque,while in the age group younger than 75 years old,fasting blood glucose and 2-hour postprandial blood glucose have a great influence on the stability of plaque,and the higher the blood glucose in a certain range,the smaller the stability of plaque.
