Objectives Although there are four commonly used methods of screening for diabetes,respectively,FPG,PG2 h,blood glucose at any time,and Hb Alc,but there are some deficiencies in these screening methods.Simple detection of fasting blood glucose misdiagnosis rate is relatively high,2 hours after the detection of blood glucose in the physical examination process is not easy to achieve,the sensitivity of glycosylated hemoglobin is not high cost is higher,the detection of blood glucose sensitivity at any time is not high.Because the detection of postprandial 1 hour blood glucose sensitivity is higher,in the physical examination process is also easier to achieve,the cost is not high,so 1 hour postprandial blood glucose may be more suitable for diabetes screening.The aim of this study was to investigate the rational use of OGTT for 1 hour postprandial glucose screening for diabetes.Methods In this study,PG2 h ? 11.1 mmol / L in the OGTT trial was selected as the gold standard for this trial in accordance with the World Health Organization [11] revised diagnostic criteria for diabetes in 1999,Whereby the selection of the diabetic group and the non-diabetic group was performed.Selected from January 2010 to December 2016,the first time in Tangshan City,North China University of Science and Technology Affiliated Kailuan General Hospital in the line of glucose tolerance test outpatient and hospitalized population,Which PG2 h ? 11.1 mmol / L of the population became diabetic group.In the same period with the crowd,PG2 h <11.1 mmol / L of the crowd to become non-diabetic group,the above population to meet the test of the inclusion criteria and exclusion criteria.And then data collection and physical examination.The method of measuring 1 hour postprandial blood glucose in this test was hexokinase..And then the data to sorting and analysis: First determine the relationship between PG1 h and diabetes,through the X~2 test to calculate the P value and OR value,followed by the area under the ROC curve to determine whether the blood glucose 1 hour after the diagnostic value,and then according to the index select the screening point,and evaluate the sensitivity,specificity of the cut-off point,and finally combine the clinical to determine the best screening of diabetic cut-off value.Results 1 sex ratio between the diabetic group and the non-diabetic group,The results showed that there was no significant difference between the two groups.2 diabetes mellitus group and non-diabetic group 1 hour postprandial blood glucose levels,the results show that: the average level of PG1 h in diabetic group was higher than non-diabetic group,the difference was statistically significant.3 The mean level of PG1 h was grouped,?14.58 as exposure group,<14.58 for non-exposure group analysis 1 hour postprandial blood glucose level and diabetes,the results showed that: diabetes and non-diabetic group between the exposed group There were statistically significant differences,OR was 50.42 and 95% Clf was 41.25 ~ 61.64.4 The correlation between PG1 h and PG2 h was analyzed,The results showed that there was a correlation between PG1 h and PG2 h,and the relationship between PG1 h and PG2 h in diabetes mellitus was higher than that in non-diabetic group.5 The correlation between PG1 h and age was analyzed,The results showed that there was a weak correlation between PG1 h and age in diabetic group and non-diabetic group.6 The relationship between PG1 h and sex was analyzed,The results showed that there was a weak correlation between PG1 h and sex in the diabetic and non-diabetic groups.7 PG1 h to screen the area under the maximum curve of diabetes was 0.949(95% confidence interval0.943 ~ 0.954,P = 0.000).8 When the PG1 h = 12.80 mmol / L,the index was 74.0%,the sensitivity was 90.8% and the specificity was 83.2%,At this time,both the higher sensitivity and the specificity were higher,the ? value was 0.67,indicating 1 hour postprandial blood glucose can be used for clinical early screening of diabetes.Conclusions PG1 h = 12.80 mmol / L can be used as a reference for screening for diabetic cut-off points.
