| ObjectiveTo explore the value of digital impedance blood flow imaging in the clinical diagnosis of elderly patients with chronic cardiac insufficiency.MethodsSelect 153 patients over 65 years old with chronic cardiac insufficiency.They are divided into groups according to NYHA,62 patients in cardiac function I-II group,58 patients in cardiac function III group and 33 patients in cardiac function IV group.At the same time,35 elderly patients without disease were selected as the healthy control group,and the patients in the study group were divided into 69 cases of coronary heart disease,39 cases of chronic pulmonary obstructive pulmonary disease,37 cases of chronic pulmonary obstructive pulmonary disease×coronary heart disease,and 8 cases of multiple causes according to the pathogenesis.WA-820digital impedance rheometer was used to detect and collect data:basic impedance Z0,impedance ventricular systolic wave amplitude HS,right precordial index Q-BI,right precordial index Q-YI,right heart failure index P2-Y,wave propagation time Q-C interval,myocardial tension index Q-B/B-Y,left ventricular diastolic function index A/C,left ventricular diastolic function index O/C and other indicators,and plasma nt-pro BNP level and color Doppler echocardiography were tested and analyzed.ResultsThere was no significant difference in age,sex and urea nitrogen between the two groups(P>0.05),and the blood creatinine and NT-proBNP were significantly higher than the control group(P<0.05).Compared with the control group,there was no significant statistical significance in the cardiac function III group.The indexes of Z0,HS,B-YI and P2-Y in the cardiac function IIIIV group were significantly decreased,and Q-BI,Q-B/B-Y and O/C were significantly increased,with statistical significance(P<0.05);D-IPR was 0.903,(95%confidence interval:0.8330.973).Three methods of NT-proBNP,cardiac color Doppler and digital impedance hemograph were combined to evaluate the state of heart function.If the test was parallel,the sensitivity was 95%,the specificity was 74%,the positive predictive value was 0.97,the negative predictive value was 0.62,and the sensitivity was 30%,the specificity was 97%,and the positive predictive value was 0.99,the negative predictive value was 0.13.ConclusionCardiac function status can be assessed by digital impedance flow imaging in elderly patients with chronic heart failure with different cardiac functions.Digital Impedance angiography combined with BNP/NT-proBNP and color Doppler echocardiography can further improve the diagnostic efficiency of cardiac function assessment in elderly patients.Digital impedance flow imaging has certain advantages in the diagnosis and differential diagnosis of chronic heart failure in elderly patients,the severity and efficacy of heart failure. |
