Objective:To investigate the relationship between oxidative stress factors such as catalase(CAT),succinate dehydrogenase(SDH),glutathione peroxidase(GsH-Px),malondialdehyde(MDA)and Chronic cardiac insufficiency.Methods: From October 1st 2016 to December 31 st 2017,135 patients(78males,54 female)patients who were continuously admitted to hospital and signed informed consent were collected in the Affiliated Zhong shan Hospital of Dalian University.The contents of the study include demographic data:age,sex,etiology,echocardiography: cardiac structure,left ventricular function and tissue doppler examination: Left ventricular systolic function was evaluated by left ventricular ejection fractionation(LVEF).Measurement of mitral flow velocity and mitral annular velocity in early diastolic phase,and calculation of E / e ' value to evaluate left ventricular diastolic function.Determination of serum CAT,SDH,GsH-Px,MDA by enzyme linked immuno sorbent assay(ELISA).Serum creatinine creatinine creatinin(Scr),blood urea nitrogenin(BUN),serum albumin(Alb)were measured.The estimation of glomerular filtration rate(eGFR)was calculated to evaluate the renal function of the patients.Determination of Aminoterminal pro-brain natriuretice peptide(NT-proBNP).At the same time,21 cases(male 11,female 10)who did not find any abnormality after physical examination in our hospital,were selected as healthy control group.The data were processed by SPSS24.0 software.One-way ANOVA analysis and Pearson Correlation analysis were used to analyze the data.Results: The results showed that the serum CAT level was 2.962±2.987 U/mgprot vs.9.915±4.461 U/ mgprot in Chronic cardiac insufficiency group and healthy control group respectively(P < 0.01).The content of serum SDH in Chronic cardiac insufficiency group and healthy control group was 0.130 ±0.025 U/mgprot vs.0.339±0.303 U/mgprot,respectively(P < 0.01).The serum GsH-Px levels in Chronic cardiac insufficiency group and healthy control group were(2.755 ±1.84)* 10 activity units vs.(2.251 ±1.889)* 10 activity units,respectively,(P>0.05).The levels of serum MDA in healthy control group and Chronic cardiac insufficiency were 2.593±1.359 nmol/mgprot vs.4.633 ±3.124 nmol/mgprot,respectively(P < 0.01).There was no significant difference in serum CAT,GsH-Px and MDA levels among different age groups in patients with Chronic cardiac insufficiency.The content of serum SDH in patients with Chronic cardiac insufficiency in 55-64 years old group and 65-74 years old group was 0.520 ±0.355 U/mgprot vs.0.297 ±0.290 U/mgprot respectively(P < 0.05).The age group of 55-64 years was significantly higher than that of the group of65-74 years old.There is no significant correlation between the content of CAT,SDH,GsH-Px,MDA and age in patients with Chronic cardiac insufficiency.The levels of serum CAT,SDH,GsH-Px and MDA in patients with Chronic cardiac insufficiency of different genders were not significantly different.There was no significant difference in serum CAT,SDH,GsH-Px and MDA levels between patients with Chronic cardiac insufficiency of different etiology.According to eGFR,there was no significant difference in serum CAT,SDH,GsH-Px and MDA in patients with Chronic cardiac insufficiency.But the content of SDH in Chronic cardiac insufficiency patients was positively correlated with eGFR,r= 0.231,p=0.007,P < 0.01.There was no significant correlation between serum CAT,GsH-Px,MDA and renal function(eGFR)in patients with Chronic cardiac insufficiency.There was no significant difference in serum CAT,SDH and GsH-Px among different LVEF classification groups.The contents of MDA in patients with different LVEF classification groups in >55% and <45%groups,were 3.943 ±2.373 nmol/mgprot vs 5.714 ±3.944 nmol / mgprot respectively(P< 0.01).LVEF < 45% group was significantly higher than LVEF > 55% group.The content of MDA in patients was negatively correlated with LVEF r =-0.238,p =0.014,(P < 0.05).The contents of c CAT,SDH and GsH-Px were not correlated with LVEF in patients with Chronic cardiac insufficiency.The contents of CAT,SDH and GsH-Px and MDA in patients were not significantly different according to the classification of E / e '.There was a negative correlation between CAT and E / e' in patients with Chronic cardiac insufficiency,r=-0.235,p=0.007.The content of MDA inpatients was positively correlated with E/e ',r=0.238,p = 0.014(P < 0.05).The content of SDH and GsH-Px in patients with Chronic cardiac insufficiency had no significant correlation with E / e'.Conclusion:LVEF is a slow change in patients with Chronic cardiac insufficiency.NT-proBNP can reflect the severity and prognosis of heart function in patients with acute heart failure and acute exacerbation of Chronic cardiac insufficiency,but it is not sensitive to Chronic cardiac insufficiency,and is affected by age,etiology,renal function and so on.It is not an ideal indicator of Chronic cardiac insufficiency.This study shows that:In patients with Chronic cardiac insufficiency,CAT was significantly lower than that in healthy persons.In the range of E/e':8-14 and E / E: > 14,CAT was negatively correlated with E / e'.These results suggest that CAT may be related to diastolic function and decrease with decrease of diastolic function.And age,sex,etiology,left ventricular systolic function(LVEF)and exercise tolerance were not associated with it.While MDA was significantly higher in patients with Chronic cardiac insufficiency than in healthy people,with a negative correlation with the value of LVEF,that is,with the decrease of LVEF,MDA in Chronic cardiac insufficiency showed an upward trend,suggesting that MDA increased with the decrease of contractile function.and It is not affected by age,sex,etiology,renal function.SDH in Chronic cardiac insufficiency patients was significantly higher than that in healthy persons,but the correlation between SDH and age,renal function was not consistent.There was no significant difference in GsH-Px content between patients with Chronic cardiac insufficiency and healthy people,and other groups.Therefore,we speculate that CAT,MDA may be an effective index to measure cardiac function.With regard to SDH,GsH-Px still needs to be expanded for further study.
