Objective:To construct the severity of electronic illness scoring information system for children with congenital heart disease after cardiac surgery,and applying and evaluating the electronic scoring system.Specific research purposes include:(1)To construct the electronic scoring system;(2)To verify the degree of consistency and efficiency of the scoring tool;(3)To analysis the level the critical early warning score and time interval.Methods:(1)Through organizing program language requirements to build the electronic scoring system;(2)By two raters using two kinds of evaluation tools 50 cases of children with congenital heart disease surgery in CICU for measuring time each score used at the same time.Used the difference between the scores,consistent rate and intraclass correlation coefficient(ICC)for two scoring tool consistency degree analysis;Used independent samples t-test,monadic linear regression analysis to analyze timeliness;(3)By applying the electronic scoring system for 607cases of children with congenital heart disease surgery CICU score per hour until the children die or CICU.Application of multiple independent samples Kruskal Wallis rank and inspection,line x(R x C)sheets data card party inspection such as general data,the rating score comparison between groups;Area under the receiver-operating characteristic curve(ROC curve)(AUC)and life table survival analysis method to granular level the critical early warning score and time interval.Results:1、Construct the severity of electronic illness scoring information system for children with congenital heart disease after cardiac surgery.2、(1)The degree of consistency verification results showed that the two scoring tool high degree of consistency:the same raters using two scoring tool for children with the same score the consistent rate of raters respectively A:94%,B:96%,the ICC were 0.993(0.988 0.996),0.990(0.983 0.995),P<0.01.Two raters used the same kind of tools for children with the same score consistent rate respectively was:print score tools,electronic scoring system,90%and 100%,the ICC were 0.984(0.972 0.991),1(1,1).(2)The timeliness validation results showed that the electronic scoring system is relatively save nearly two-thirds of the time.Monadic linear regression analysis results:the regression model of time and score points score of the paper score tool was:Y=2.398+46.049 X,R~2=0.790,and the regression model of time and score points score of the electronic scoring system was:Y=0.418+19.829 X,R~2=0.058.3、Refined the 5 early warning score and time interval of each critical grades:“I-no risk”“II-mild critically ill”“Ⅲ-moderate critically ill”“IV-severe critically ill”“V-extremely critical”.Conclusion:(1)Constructed the electronic scoring system which highly restored the paper version scoring tool,and with higher accuracy and less affected by human factors.(2)The electronic scoring system have advantage on time.(3)Analysised 5 risk levels of critical early warning score and the corresponding time interval.The full use of the electronic information technology,through the real-time dynamic rating,assess critically ill children after minds first degree change trend to predict and judge the prognosis of children with postoperative risk.
