Objectives:1.Knee Osteoarthritis(KOA)model rats were used as the research object to detect the acupoint sensitization phenomenon of the model rats;2.To study the therapeutic effects of electro-acupuncture sensitized acupoints on KOA model rats and the regulation effects of local inflammatory factors in joints.Methods:1.Experiment One:56 SD rats were randomly divided into blank group and model group according to the ratio of 1:3,of which 14 rats were in blank group and42 rats in model group.The model group was made by MIA joint cavity injection,while the blank group was injected with the same amount of NS.Electronic Von Frey algesimeter and Infrared Thermal Camera were used to detect the mechanical pain threshold and acupoint temperature of 13 acupoints including ST34,ST35,ST36,SP6,SP9,SP10,GB34,GB39,LR3,BL23(L and R),EX-LE2,EX-LE4 of rats before and after the model was established.The change rate of mechanical pain threshold and acupoint temperature was calculated.The high-sensitivity and low-sensitivity acupoints were determined according to the change rate.Calculate the probability of high sensitivity or low sensitivity of each acupoint and analysis of the distribution of high-sensitivity and low-sensitivity acupoints on rats.2.Experiment Two:36 SD rats remained from experiment one,of which 9 rats were in blank group and 27 rats in model group.According to the change rate of mechanical pain threshold of points on model rats.EX-LE2 s are both high probability of high sensitization(27%,9)and high probability of low sensitization(27%,9).EX-LE2 is selected as the observation point,9 rats with high sensitization of EX-LE2 are taken as the high-sensitivity group,9 rats with low sensitization of EX-LE2 are taken as the low-sensitivity group,the rest 9 rats are taken as the model control group.For EX-LE2,the non-meridian non-point point 5mm up straight from EX-LE2 is selected for electro-acupuncture matching point.After 2 courses of electro-acupuncturetreatment,Lequesne MG score,the joints diameter and plantar pain threshold of rats were evaluated.HE staining and microscopic observation of cartilage histopathology Mankin's score;Western Blot was used to detect the expression of ChemR23 protein in cartilage tissue,ELISA was used to detect the concentrations of chemerin,IL-6 and TNF-? in irrigation fluid of joint cavity,and the effects of electro-acupuncture at high-sensitivity and low-sensitivity acupoints on the above indexes were analyzed.Results:1.The three acupoints with the highest probability of high pain sensitivity in rats are SP9(36.4%),SP6(33.3%)and ST36(33.3%);The three acupoints with the highest probability of low pain sensitivity are BL23-R(51.5%),LR3(39.4%),and BL23-L(33.3%).2.The three acupoints with the highest probability of high temperature sensitivity are LR3(48.5%),GB39(36.4%)and BL23-L(36.4%).The three acupoints with the highest probability of low temperature sensitivity are BL23-R(42.4%),SP9(39.4%),GB39(33.3%).3.Lequesne MG score: after intervention,the score of the low-sensitivity combined high-sensitivity group was significantly lower than that of the model control group(P<0.05);Compared with the low-sensitivity group,the score of the high-sensitivity group decreased,but the difference was not statistically significant(P>0.05).Suggested that electro-acupuncture can improve the behavior Lequesne MG score of model rats,and the improvement effect of electro-acupuncture at high-sensitivity points may show a more obvious improvement trend.4.Joint diameter: after intervention,the joint diameter of high-sensitivity group was significantly smaller than that of model control group and low-sensitivity group(p < 0.05);Compared with the model control group,the joint diameter in the low-sensitivity group has a decreasing trend,but the difference is not statistically significant(P>0.05).Suggested that electro-acupuncture at high-sensitivity points has a more obvious effect on improving the joint diameter in the model rats.5.Plantar pain threshold: compared with the model control group,the pain threshold of the low-sensitivity group and the high-sensitivity group increasedsignificantly after the intervention(P<0.05).Compared with the low-sensitivity group,the high-sensitivity group had no significant difference(P>0.05).Suggested that electro-acupuncture can increase the plantar pain threshold of rats,but electro-acupuncture at high-sensitivity and low-sensitivity points has no difference in improving the pain threshold of rats.6.Cartilage Mankin's score: Mankin's scores of low-sensitivity group and high-sensitivity group were significantly lower than those of model control group(P<0.05),while Mankin's scores of high-sensitivity group were significantly lower than those of low-sensitivity group(P<0.05).Suggested that electro-acupuncture can improve Mankin's scores of model rats,and the improvement effect of high-sensitivity points is more obvious.7.Concentration of chemerin in joint fluid: compared with model control group,the concentration of chemerin in high-sensitivity group decreased significantly(P<0.05),while that in low-sensitivity group decreased,but the difference was not statistically significant(p>0.05).Compared with the low-sensitivity group,the chemerin in the high-sensitivity group decreased,but the difference was not statistically significant(P>0.05).It is suggested that acupuncture can reduce the concentration of chemerin in joint fluid of model rats,and the down-regulation effect of highly sensitive acupoints may be more obvious.8.Cartilage chemR23 protein expression: compared with the model control group,the expression of cartilage chemR23 protein in the low-sensitivity group and the high-sensitivity group showed a downward trend,but the difference was not statistically significant(p>0.05).Compared with the low sensitivity group,the protein expression showed a decreasing trend,but the difference was not statistically significant(P>0.05).It is suggested that electro-acupuncture may down-regulate the expression of ChemR23 protein in cartilage tissue of model rats,and the down-regulation effect of highly sensitive acupoints may be more obvious.9.The concentration of IL-6 in joint fluid: compared with the model control group,IL-6 in high-sensitivity group decreased(P<0.05),while that in low-sensitivity group decreased,but the difference was not statistically significant(p>0.05).Compared with the low-sensitivity group,IL-6 in the high-sensitivity group decreased(P<0.05).It is suggested that electro-acupuncture can reduce the concentration of IL-6 in the joint fluid of model rats,and the effect of high sensitive points is more obvious.10.The concentration of TNF-? in joint fluid: compared with the model control group,TNF-? in the low-sensitivity group and the high-sensitivity group has no statistical difference(P>0.05),and the difference between the high-sensitivity group and the low-sensitivity group has no statistical significance(P>0.05).Suggested that electro-acupuncture has no obvious regulation effect on TNF-? concentration in joint fluid of rats in this experiment.Conclusion:1.Acupoint sensitization phenomena exist in KOA model rats: hyperalgesic acupoints are mainly joint local acupoints and hypalgesic acupoints are mainly distal acupoints.The temperature of high-sensitivity and low-sensitivity acupoints were mainly located at the distal acupoints.2.Electro-acupuncture can improve the joint diameter and cartilage Mankin's score of model rats,and the improvement effect of electro-acupuncture on hyperalgesic points is more obvious.3.Electro-acupuncture can improve the Lequesne MG score and plantar pain threshold of KOA model rats;4.Electro-acupuncture can reduce the levels of chemerin and IL-6 in rat joints,and the effect of electro-acupuncture on hyperalgesic points is more obvious.
