| Objective:To compare the clinical characteristics of patients in COPD and COPD with bronchiectasis in the acute exacerbation stage.To investigate the distribution of TCM syndromes of COPD with bronchiectasis and its correlation with pulmonary function and etiology.Methods:Retrospectively collected the case data of patients diagnosed with AECOPD.According to HRCT,the patients were divided into COPD group and COPD with bronchiectasis group.SPSS19 software package was used for data analysis.Results:1.A total of 182 patients with acute exacerbation of COPD were included in this study,including 58 patients in the group COPD with bronchiectasis(group A)and124 patients in the COPD group(group B).There were significant difference between the two groups in gender,BMI,course of disease and length of hospitalization,smoking status and age was not.2.The two groups have a significant difference between WBC,N%,CRP and PCT.And there was no significant difference in PaO2 and PaCO2.3.The ratio of severely impaired pulmonary function in group A was higher than that in group B.FEV1%pred was significantly different between the two groups.4.The proportion of expectoration and sputum in group A was higher than that in group B.The positive rate of sputum culture in group A was higher than that in group B.The infection rate of pseudomonas aeruginosa in group A was higher than that in group B,and the difference was statistically significant.5.The bronchiectasis were most in the following lobes and usually involving both lungs.6.The distribution of TCM syndromes in the two groups was not completely the same.The phlegm-heat stagnation lung syndrome and phlegm-turbidity obstruction lung syndrome had statistical significance in two groups.7.There were differences in the grading of lung function severity between different TCM syndromes,and with significant differences in the level of FEV1%Pred.In patients with branchiectasis,there was no significant difference in the grading of pulmonary function severity between syndromes,with the lowest FEV1%pred in phlegm-heat stagnation lung syndrome.Phlegm-heat stagnation of lung syndrome was the most common syndrome to colonization or infection of pseudomonas aeruginosa,with statistical significance.There was the highest proportion of colonization or infection by pseudomonas aeruginosa of Phlegm-heat stagnation of lung syndrome in group A,and the difference was not significant.Conclusion:1.COPD patients with bronchiectasis have stronger inflammatory response,longer inpatient treatment,poorer lung function,and greater chronic consumption.2.Severe chronic obstructive pulmonary disease patients have a high prevalence of bronchiectasis in HRCT,and the co-existence of moderate to severe COPD and bronchiectasis indicates that patients have severe airflow obstruction,which is more serious.3.Lower respiratory tract bacterial colonization is more easily formed in COPD with bronchiectasis,among which pseudomonas aeruginosa is the most common.4.Chronic obstructive pulmonary disease(COPD),which is characterized by phlegm-heat stagnation of the lungs,may indicate a greater tendency to be associated with bronchiectasis.Which the lung function is worse,Lower respiratory tract colonizers are more likely to exist due to bronchiectasis,and pseudomonas aeruginosa is the most common colonizer. |
