Objective To explore the clinical characteristics of COPD patients complicated with bronchiectasis,so as to improve the understanding and diagnosis of the disease.Methods 130 cases of AECOPD patients from January 2015 to December 2015 in our hospital were selected as the research object,which included 22 cases of COPD patients complicated with bronchiectasis(the study group)and 108 cases of COPD patients(the COPD group).The general condition of the patients,white blood cell count,C-reactive protein,procalcitonin,fibrinogen,serum albumin,the results of bacterial culture of sputum,lung function tests and arterial blood gas analysis results were collected.And then the differences in clinical features,lung function and laboratory tests between the two groups were compared.Results1.This study included 130 cases of AECOPD patients.The study group included 22cases(occupied 16.9%),of which 8 were male,14 were female,aged 43-85 years old,the average age was(64.2±12.2)years,the course of disease was(16.5±5.8)years,the smoking index was(283.2±72)packs of cigarettes/ year;and the COPD group included108 cases,72 males and 36 females,aged 50-91 years old,the average age was(67.5±9)years,the course of disease was(18.2±3.2)years,the smoking index was(250.6±66.8)packs of cigarettes/ year.The general condition of the two groups were not statistically significant(P > 0.05).2.The dyspnea score(m MRC)was higher in the study group(2.36±0.7)than the COPD group(1.93 ± 0.6),the difference was statistically significant(t=2.87,P < 0.01).The proportion of daily purulent sputum in the study group accounted for 90.9%,the incidence of pulmonary heart disease accounted for 68.2%,both were significantly higher than those in COPD group(P < 0.05).The average length of hospital stay in the study group(11.9±5.7 days)was significantly higher than that in the COPD group(9.0±3.2 days),the difference was statistically significant(t=2.31,P < 0.05).3.The decline of FEV1%pred,FEV1 /FVC,FVC% pred and DLCO%pred were more pronounced in the study group than those in the COPD group,the difference was statistically significant(t =-4.37,-2.76,-4.24,-3.24,P < 0.01).The incidence of hypoxemia in the study group was higher than that in the COPD group,the difference was statistically significant(P < 0.05).4.The levels of C-reactive protein,procalcitonin,albumin and fibrinogen in the study group were significantly higher than those in COPD group,the difference was statistically significant(t = 2.66,2.25,2.77,P < 0.05);the albumin level in the study group(31.2 ± 2.5 g/L)was significantly lower than that in the COPD group(32.9 ±2.5g/L),the difference was statistically significant(t=-2.97,P < 0.01).5.The times of acute exacerbation during the 6 months follow-up in the study group(1.68±0.48)was significantly higher than that in the COPD group(1.28±0.45),the difference was statistically significant(t= 2.97,P < 0.01).The mortality rate was 27.3%in the study group,significantly higher than that in the COPD group,the results showed significant difference(P < 0.05).Conclusion COPD patients complicated with bronchiectasis show a more severe condition,stronger inflammatory response,longer course of treatment,lower lung function,and have a higher risk of exacerbation.In clinical,for patients diagnosed with COPD,high resolution CT scan should be routinely performed to early diagnosis and early treatment,so as to improve the prognosis of patients.
