ObjectivesThis study applied the Traditional Chinese Medicine nursing based on mobile platform to patients with primary insomnia,aimed to explore a remote intervention method for Traditional Chinese Medicine nursing which includes self-assessment,health education,remote guidance,diet therapy,traditional exercise therapy and five-elementary music therapy,it is helpful to achieve long-term monitoring and care outside the hospital for patients with insomnia.In order to improve the sleep quality and mental status of insomnia patients,save patients'time and economic costs,optimize the integration of medical resources and provide valuable practical experience,as well as provide an example for other out-of-hospital health management of chronic diseases and the exploration of intelligent medical service mode under the background of"Internet+".MethodsUsing the methods of literature review and expert consultation,we build a Traditional Chinese Medicine mobile platform of primary insomnia,and put the Traditional Chinese Medicine nursing mobile platform to use.We conducted a randomized controlled trial.From March 2018 to February 2019,84 patients with primary insomnia were recruited according to inclusion and exclusion criteria in a top 3 TCM hospital in Chengdu.Patients with primary insomnia were randomly divided into experimental group?using TCM nursing intervention based on mobile platform?and control group?routine health education?by using a random number table,42 patients in each group.The control group was given routine health education for 8 weeks,and the manual about the basic knowledge of insomnia and health promotion was distributed to every patients with primary insomnia.The on-site guidance and PPT lectures were used for health education,once every 2 weeks,30 minutes each time,and individualized guidance was implemented face-to-face or using telephone,text message and WeChat,once a week,5-10 minutes each time.On the basis of routine health education,the experimental group conducted a Traditional Chinese Medicine nursing intervention with mobile platform for 8 weeks.As for diet,according to the syndrome differentiation of insomnia and personal preferences,patients were advised to take the Chinese medicine diet once a day according to the recipe.In terms of five-elementary music,patients can choose the five-elementary music according to the syndrome differentiation of insomnia and personal preferences,and it was recommended to listen to five-elementary music at least once a day,about 30 minutes each time,the volume range was controlled to40-60 decibels.As for exercise therapy,collective teaching and individualized guidance was conducted in Saturday afternoon,patients with primary insomnia can use TCM nursing mobile platform or WeChat platform to study and exercises in free time,at least once a day,about 35 minutes each time,lasting 8 week.Pittsburgh Sleep Quality Index?PSQI?,Insomnia Severity Index?ISI?,The Mos 36-item Short Form Health Survey?SF-36?,Self-rating Anxiety Scale?SAS?,and Self-rating Depression Scale?SDS?were used to evaluated sleep quality,severity of insomnia,quality of life,and the status of anxiety and depression were recorded before intervention and after 8weeks intervention.The data were analyzed by SPSS21.0 statistical analysis software.The measurement data conforms to the normal distribution were expressed as meanąstandard deviation?x?s?,and two independent sample t tests were adopted.The measurement data conforms to the skewed were described with median?quartile?[M(P25,P75)],non-parametric Mann-Whitney U rank sum test were used;the count data was described by the frequency and composition ratio,and the chi 2test was used in the comparison of count data.ResultsA total of 94 subjects were included in the study.The final 84 patients successfully completed the intervention,42 patients in experimental group?1 case was excluded,4 cases were lost?,and 42 cases in control group?2 cases were excluded and 3 cases were lost?.1.Baseline information:before the intervention,the difference of the general data,the PSQI scores,the ISI scores,the SF-36 scores,the SAS scores and the SDS scores of two groups were not statistically significant?P>0.05?,so they had comparability.2.Two groups comparison after intervention:after receiving different interventions,the PSQI total scores and the scores of each factor,the ISI score,the SF-36 dimension,the SAS score and the SDS score were statistically significant?P<0.05?.After the intervention,the clinical efficacy was compared,efficiency of the control group was 83.33%,while the experimental group was 92.86%.3.Within group comparison before and after the intervention:before and after the intervention,the PSQI scores,ISI scores,SF-36 scores,SAS scores and SDS scores of experimental group were statistically significant?P<0.05?.The PSQI total score,ISI score,SAS score and SDS score of control group before and after the intervention were statistically significant?P<0.05?.There were significant differences in sleep quality,time of going to sleep,sleep time,and daytime dysfunction among PSQI scores of each factor?P<0.05?.There were significant differences physical function,general health and role-emotional in all dimensions of SF-36 scores?P<0.05?.ConclusionThe Traditional Chinese Medicine Nursing based on Mobile platform can effectively improve sleep quality,severity of insomnia,quality of life,and status of anxiety and depression.Traditional Chinese Medicine nursing intervention was conducted by diet therapy,exercise therapy and five-elementary music therapy,which are economical and effective and can be applied in clinical practice.
