| Objective: Hypertension is the main cause of cerebral infarction,and the number of patients with cerebral infarction complicated with hypertension is dramatically increasing.The study aimed to explore the clinical value of leukocyte-related indexes as biomarkers in acute stage of cerebral infarction complicated with hypertension.Methods:According to the past history of hypertension,acute cerebral infarction patients who were hospitalized in the sixth Department of Neurology,second Hospital of Hebei Medical University were divided into simple cerebral infarction(CI)group and cerebral infarction complicated with hypertension(CIHT)group.The peripheral blood leukocyte-associated and biochemical indexes were collected on admission.National Institutes of Health Stroke scale(NIHSS),modified Rankin Scale(mRS)and Barthel Index(BI)at Day 0,Day 7,Day14 and Day 90 were performed.Results: There was no significant difference in age,sex,smoking,drinking and BMI index between CI group(n=53)and CIHT group(n=64)(P > 0.05).1.There was no significant difference in stroke scale scores between the two groups on admission(P > 0.05).Compared with CI group,NIHSS scores were higher at Day 90,mRS scores were higher at Day 14 and Day 90,as well as BI scores were lower at Day14 in CIHT group(P < 0.05).2.Compared with CI group,neutrophil count(NE),neutrophil ratio(NEUR)and neutrophil/lymphocyte ratio(NLR)increased significantly,platelet count(PLT),platelet/leukocyte ratio(PWR)and platelet/neutrophil ratio(PNR)decreased significantly in CIHT group(P < 0.05).3.CIHT patients were associated with NE,NEUR,PLT,PWR,PNR,low HDL,and high HCY,which were correlated with short term prognosis.Multi-element regression analysis showed that only PNR,high HCY were independent risks for CIHT patients(P < 0.05).Conclusion:1.Hypertension had no effect on the early stage,but is related to neurological function recovery for patients with acute cerebral infarction.2.CIHT patients had significantly high level of NE,NEUR,NLR and low level of PWR,PNR,PLT in the peripheral blood on admission.3.It was suggested for the first time that high level of NE,NEUR and low level of PLT,PWR and PNR are risk factors for CIHT patients.4.Compared with NE,NEUR,PLT and PWR,PNR might be used as a biomarker for the diagnosis and prognosis of CIHT patients in acute phase. |
