Objective: To investigate the incidence of diabetic kidney disease in patients with type 2 diabetes mellitus,and to analyze the related risk factors,so as to provide theoretical support for the clinical prevention and treatment of diabetic kidney disease.Methods: A total of 238 T2 DM patients were selected from the endocrinology department in third hospital of hebei medical university from April 2018 to August 2018,including 151 males and 87 females.According to the urinary albumin creatinine ratio,the patients were divided into simple diabetes mellitus group(SDM group: UACR < 30mg/g)and diabetic kidney disease group(DKD group: UACR ≥30mg/g).Sex,age,height,weight,course of diabetes mellitus,smoking history,drinking history,family history,blood pressure,BMI,fasting blood glucose,2-hour postprandial blood glucose,glycosylated hemoglobin,total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol,serum uric acid,creatinine,blood urea nitrogen,C-reactive protein,fibrinogen,thyroid stimulation Hormone and urinary albumin/creatinine were collected.SPSS 23.0 statistical software was used for statistical analysis.Counting data were expressed as rates(%),and chi-square test was used.Data with normal distribution were expressed as mean±standard deviation(?x ± S),non-normal data were showed by the median(interquartile range).T-test was performed for those who obeyed normal distribution,and rank sum test was used for those who did not obey normal distribution.In order to exclude the interaction of various factors,the independent risk factors of DKD were analyzed by multivariate logistic regression,and the occurrence of diabetic kidney disease was the dependent variable.P<0.05 was defined as inclusion criteria.Results:1.Comparison between SDM and DKD : The age of DKD group(62.40±11.34)years was higher than that of SDM group(56.14±12.17)years(t=-3.888,P<0.01).SBP[148(28)mmHg]in DKD group was higher than SBP[135(20)mmHg] in SDM group(Z=-4.363,P=0.000013).Course of disease [12.5(14)years] in DKD group was higher than Course of disease [9(12)years]in SDM group(Z=-3.401,P=0.001).HbA1c[8.7(2.4)%] in DKD group was higher than HbA1 c [8.2(2.4)%] in SDM group(Z=-2.427,P=0.015).FIB [3.36(0.77)g/L] in DKD group was higher than FIB[3.36(0.77)g/L] in SDM group(Z=-2.015,P=0.044).There were significant differences in age,SBP,course of disease,HbA1 c and FIB between the two groups.2.Spearman correlation analysis showed that age(r=0.281,P<0.01),course of disease(r=0.281,P<0.01),SBP(r=0.322,P<0.01),TSH(r=0.128,P<0.05),FIB(r=0.18,P<0.05),gender(female)(r=0.136,P<0.05)and UACR were positively correlated.3.Multivariate logistic regression analysis showed age(B = 0.367,OR = 0.367,P < 0.05),the course of disease(B = 0.397,OR = 1.487,P < 0.05),SBP(B = 0.669,OR = 1.953,P < 0.01),HbA1c(B = 0.472,OR = 1.604,P < 0.05).Age,course of disease,SBP and HbA1 c were independent risk for diabetic kidney disease.Conclusion:1.Age,course of disease,systolic blood pressure,TSH,FIB,female were closely related to DKD.2.Age,course of disease,SBP and HbA1 c were independent risk factors for DKD.
