Purpose:Sema4D(Sema4D)is a family member of transmembrane and secretory proteins.During the process of platelet activation,it splits and falls off,enters the blood circulation,and becomes serum soluble Sema4D.Related studies have shown that Sema4D is closely related to inflammation,platelet activation andatherosclerosis.Diabetic nephropathy is one of the microvascular complications of type 2 diabetes mellitus.It is an important cause of end-stage nephropathy.Its occurrence and development are related to endothelial cell damage and platelet activation.At present,the treatment is limited and the prognosis is poor.At present,urinary microalbumin creatinine ratio(ACR)is often selected as the clinical indicator of diabetic nephropathy stage.The purpose of this study is to explore the relationship between serum signal element 4D(Sema4D)level and urinary microalbumin creatinine ratio(ACR)in patients with type 2 diabetes mellitus,so as to provide a new predictive index for microvascular complications in patients with type 2 diabetes mellitus and guide the treatment of diabetic nephropathy.Methods:We included 135 type 2 diabetes patients in our study.24-hour urine samples were collected to detect the urinary microalbumin creatinine ratio(ACR).Type 2 diabetes patients with ACR?30mg/g were included in the " diabetic group"(n=61),and type 2 diabetes patients with ACR>30mg/g were included in the "diabetic nephropathy group"(n=74).The general situation and clinical biochemical indexes of the patients were recorded.The serum level of Sema4D was detected by ELISAConsequences:1.The concentration of sSema4D in the diabetic nephropathy group(3.71± 0.18)UG/1 was significantly higher than that in the diabetic group(3.45±0.30)UG/L(P<0.001).2.Among all the researchers,Sperman correlation analysis showed that the level of sSema4d was positively correlated with age(P=0.008)andurinary microalbumin creatinine ratio(P=0.000),however,sSema4d was negtaively correlated withe-GFR(P=0.000)and HDL(P=0.000).3.Among all the patients included in the study,whether or not diabetic nephropathy(0 for non diabetic nephropathy,1 for diabetic nephropathy)was taken as the dependent variable,and the results of logistic regression analysis showed that sSema4d(or=1.95 P=0.000)is an independent risk factor affecting the occurrence of diabetic nephropathy.Conclusions:1.Patients with type 2 diabetes mellitus and diabetic nephropathy have higher serum level of Sema4D than those without diabetic nephropathy.2.In patients with type 2 diabetes mellitus,the concentration of sSema4d was positively correlated with age andurinary microalbumin creatinine ratio.3.Sema4D is an independent risk factor of type 2 diabetic nephropathy,which may play a synergistic role in the development of diabetic nephropathy.
