Objective1.To analyze the clinical characteristics of type 2 diabetes mellitus(T2DM)patients with different levels of urinary albumin / creatinine ratio.2.To study the influencing factors of urinary albumin creatinine ratio(UACR)in patients with type 2 diabetes mellitus.3.To study the correlation between different levels of urinary albumin / creatinine ratio and chronic complications of type 2 diabetes mellitus,and to provide reference for early diagnosis and treatment of diabetic nephropathy(DN)and diabetic retinopathy(DR).MethodsFrom November 2021 to May 2022,424 patients with T2 DM were collected from the First Affiliated Hospital of Dali University and Dali Prefecture People ’s Hospital,including 257 males;167 women.The general data,biochemical indexes and diabetic complications were collected.Patients were divided into three groups according to UACR : normal UACR group(UACR < 30 mg / g),micro UACR group(30mg / g ≤UACR < 300 mg / g)and large UACR group(UACR ≥ 300 mg / g).There were 275 cases in normal UACR group,113 cases in micro UACR group and 36 cases in large UACR group.SPSS25.0 software package was used to analyze the above data.Results1.Compared with the normal UACR group,the course of diabetes,the course of hypertension,body weight,FBG,PBG,HbA1 c,UA,SBP,DBP,Cr,WBC,NEUT,FIB,GFR and TG in the large UACR group were significantly increased,and 25-OH-D3 and TBIL were significantly decreased.2.DBP(b = 9.632,t = 2.426,P = 0.028),FBG(b = 27.324,t = 2.277,P = 0.023),HbA1c(b = 20.769,t = 2.249,P = 0.025),UA(b = 1.00,t = 2.855,P = 0.014),NLR(b = 6.484,t = 1.544,P = 0.014)were the influencing factors of UACR,and the difference was statistically significant(P < 0.05).3.The incidence of DR,DPN and diabetic carotid artery disease in the large UACR group was significantly higher than that in the micro UACR group and the normal UACR group.4.The effect of UACR on DR(OR = 1.667,95 % CI 1.067-2.605,P = 0.025)and diabetic carotid artery disease(OR = 2.173,95 % CI 1.343-3.516,P = 0.002)was statistically significant(P < 0.05).UACR is an independent influencing factor for DR,DN and diabetic carotid artery disease.Conclusion1.Patients in the large UACR group had longer duration of diabetes and hypertension,higher levels of SBP,DBP,FBG,PBG,HbA1 c,APOA,UA,NLR,FIB,NEUT,and WBC,and lower levels of 25-OH-D3.2.The course of disease,DBP,FBG,HbA1 c,UA and NLR were the influencing factors of UACR.25-OH-D3 is a protective factor for UACR.3.UACR level is a risk factor for DR and diabetic carotid artery disease.
