Objective: In this study,a randomized,parallel-controlled,double-blind,prospective study was conducted to explore the clinical effect and safety of hydrogen inhalation in the patients with acute exacerbation of chronic obstructive pulmonary disease.Method: 42 AECOPD patients admitted to the First Hospital of Hebei Medical University from March 2016 to December 2017 were enrolled in the study. The patients were randomly assigned to the study group and the control group in the ratio 1:1,with 21 patients in each group. The control group patients were treated with 33% oxygen inhalation. In the same way,The study group were treated with 33% oxygen and 66.7% hydrogen inhalation, all patients were inhaled twice a day for 1 hour,3 L/min for 12 days. From the baseline period to the 12 th day of the test, clinicians and technician made ward round and observed thees patients' condition every day.Blood routine, liver function, renal function, myocardial enzymes, and CRP were taken in the baseline of the test and the morning of the 12 th day.The clinical effect and safety of hydrogen therapy were evaluated by comparing the data.Result: Blood routine: Compared with the data before treatment, the erythrocyte count and hemoglobin in the study group decreased after change after treatment(P > 0.05),and the percentage of neutrophils decreased after treatment(P < 0.05).Liver and kidney function: Compared with the data before treatment, total protein, albumin and creatinine in the study group decreased significantly after treatment(P < 0.05),and creatinine clearance rate increased significantly(P < 0.05);total protein, albumin, creatinine and creatinine clearance rate in the control group had no change after treatment(P > 0.05).Myocardial enzymes: Compared with the data before treatment, creatine kinase in study group decreased after treatment(P < 0.05),while creatine kinase in control group have no change after treatment(P > 0.05).C-reactive protein: Compared with the data before treatment, there was no change after treatment in the study group(P > 0.05),but decreased in the control group(P < 0.05).Conclusion:High-concentration hydrogen therapy can reduce blood hemoglobin levels in AECOPD patients and improve renal function and muscle tissue metabolism in patients. There was no obvious adverse reaction in the treatment process, but it can lead to the decrease of albumin level. It is worth studying whether the long-term effect leads to malnutrition in patients.
