Objective:To observe and evaluate the clinical effect of chyle fat transplantation on the aged patients with dorsal hand skin,and summarize the injection level and operation skills of chyle fat filling dorsal hand skin,so to explore the mechanism of chyle fat in volume filling.Methods:From March 2016 to August 2018,subcutaneous chylous fat transplantation was performed on the dorsum of hand in 18 aged female beauty seekers.Under general intravenous anesthesia or block anesthesia,subcutaneous fat was absorbed by swelling anesthesia technology through 20 ml syringe under low negative pressure.The donor area preferred the inner thigh or lower abdomen.The obtained fat was removed by gauze filtration after the water was removed in the syringe.The swelling fluid,broken fat cells and fibrous connective tissue were removed by gauze filtration until the fat became gelatinous.Two 20 ml syringes were connected to the "three-hole fat cutter" and injected repeatedly for 5 times.The filtered fibrous connective tissue such as fat capsule was discarded.The prepared chyle fat was separated into 1 ml syringe.23G blunt needles were connected to the superficial fat layer on the dorsum of the hand,and 27G sharp needles were connected to the subdermal layer on the wrinkles.The chyle fat was filled 20-40 ml on each dorsum of the hand.Among them,12 cases were filled once,6 cases were filled twice,the interval period was 3 months,and the longest follow-up period was 18 months.The patients were followed up regularly after operation,and standard photographs were taken to investigate their satisfaction.Result:Chyloid fat transplantation on the back of the hand can effectively improve the insufficient tissue volume and increase the volume of subcutaneous tissue.Follow-up for 3 to 18 months showed that 16 of the 18 beauty seekers had different improvements in dorsal fullness and skin texture,and 2 had no effect.Patient satisfaction was 77.8%.There were no complications such as infection,embolism,fat liquefaction,displacement,granuloma and skin inequality in 18 cases of beauty seeking.SPSS16.0 statistical software package was used for data sample analysis.The calculated frequency distribution table was used for sample data.Variance analysis was used for comparison among sample groups.P<0.05 was of statistical significance.Composition ratioThe "Regulations" focus on the number of pre-operative data samples,the observation unit and the ratio of the proportion baseNumber of patients with Billiards Age Physical Activity Nutritional Status Hand Back Status Hypodermic elasticity decreases 9 32-62 31.25 21.43 52.68 Subcutaneous collagen reduction 9 32-62 29.50 28.31 57.81 Connective tissue atrophy 9 32-62 25.00 14.69 39.69 Elastic fiber reduction 9 32-62 14.25 35.57 49.82Relative ratioOdds ratio(OR)is used for clinical observation of case-to-case ratio and control study data.In the study of hand annualization therapy,when the fat acquisition method and fat preparation are unified,i.e.the filling average unit is 30.6%ML,the final sample data are as follows.Grouping Subcutaneous Elasticity Decreases Subcutaneous Collagen Decreases Connective Tissue Decreases Elastic Fiber Decreases Satisfaction%Soft tissue volume 19.03 22.15 17.87 18.75 77.8 Fine lines 12.15 17.72 19.46 17.37 66.7 Skin texture 6.11 11.39 9.51 6.03 33.4Conclusion:1.Autochylous transfer and dorsal hand transplantation can effectively improve the aging condition of dorsal hand skin.It can improve the volume of subcutaneous soft tissue to a certain extent and help to improve the texture and fine lines of skin2.Young and middle-aged patients with good nutritional status were better than elderly patients and patients with poor nutritional status...
