Background:In recent years,the number of HIV/AIDS cases in the elderly has been increasing,and relatively few studies have been made on HIV/AIDS in the elderly.Yixing is located at the southern tip of Jiangsu province and the center of the Shanghai-Nanjing-Hangzhou Triangle.The population is highly mobile.Various risk factors that cause the widespread AIDS epidemic are common,and the public's awareness of protection is still weak.In particular,since the first case of elderly people with AIDS over the age of 50 was discovered in 2009,the incidence of HIV/AIDS in the elderly in Yixing has been increasing in recent years.In order to strengthen the prevention and control of HIV/AIDS in the elderly,this study analyzes the prevalence of HIV/AIDS in the elderly in Yixing from 2009 to 2016 and identifies the risk factors affecting HIV/AIDS in the elderly in Yixing.And further carry out AIDS health education in the elderly HIV/AIDS cases,analyze the effects of intervention,study the existing problems and deficiencies,provide basic data for the control of AIDS in the elderly,and provide preliminary research on prevention and control measures.Objective:1.To understand the prevalence trends and epidemiological characteristics of HIV/AIDS in the elderly in Yixing.2.Analyze the risk factors affecting HIV/AIDS in the elderly.3.Effect of intervention on the change of knowledge and behavioral characteristics of AIDS after the intervention of HIV/AIDS cases in the elderly.Methods:1.Investigation of the status of 131 cases of HIV/AIDS in the elderly aged 50 years and over discovered in Yixing between 2009 and 2016.A descriptive analysis of three distribution characteristics,transmission pathways,exposure history,detection pathways,marital status,etc.2.A case-control study was carried out on 131 cases of HIV/AIDS in the elderly aged 50and above and 200 cases of ordinary residents by age and sex in Yixing.The risk factors of HIV infection in the elderly were analyzed using the single factor and multiple factor Logitic regression model.3.Conduct health education and psychological intervention on AIDS-related knowledge in all elderly HIV/AIDS cases aged 50 and above when confirmed,followed by follow-up intervention every six months for HIV-infected patients and every three months for AIDS patients.The intervention time was at least one year.102 cases of HIV/AIDS aged 50 years and over after the intervention were compared with the intervention before the intervention in terms of awareness,attitude,sexual behavior,and condom use.Results:1.The trend of HIV/AIDS among the elderly in Yixing is increasing year by year.101males and 30 females reported HIV/AIDS cases in the elderly aged 50 and above,with a sex ratio of 3.37:1;The Han nationality is the main;The largest 85 years of age,of which the 50-59age group is the majority,accounting for 49.6%;Marital status is predominantly unmarried/divorced or separated/widowed;Junior high school education and below accounted for 89.3%;The occupation is dominated by farmers,accounting for 73.3%;The sample source is mainly medical institutions,accounting for 86.2%;Elderly HIV/AIDS cases are geographically concentrated,with Guanlin Town accounting for 55%.2.Risk factors for AIDS in the elderly in YixingMultiple factor Logitic regression results show:unmarried or divorced/widowed(OR=9.272,95%CI:4.253~20.214),sexual history(OR=11.617,95%CI:3.721~36.263),Commercial sex(OR=3.397,95%CI:1.129~10.224),same-sex sex(OR=6.015,95%CI:1.367~26.470),non-marital sex does not always use condoms(sometimes/not used)(OR=17.972,95%CI:2.66~122.182)is a risk factor for HIV infection.3.As of January 2018,a total of 102 elderly HIV/AIDS case interventions entered the intervention group for at least one year.The awareness of AIDS-related knowledge before intervention group was 69.9%,and 93.0%after intervention.The difference was statistically significant(?~2=180.855,P<0.05);The number of people with positive attitudes towards AIDS before intervention accounted for 9.8%,and 39.2%after intervention.The difference is statistically significant(?~2=23.844,P<0.05);In the last year before the intervention,23.5%of non-marital non-commercial sex occurred,and 5.9%after the intervention.The difference is statistically significant(?~2=12.662,P<0.05);Commercial sex in the last year before the intervention accounted for 39.2%,and 2.0%after the intervention(?~2=43.295,P<0.05);In the last year before the intervention,22.5%of men and men had same-sex sex,and 6.9%after the intervention.The difference between the two groups was statistically significant(?~2=12.662,P=0.002);The use of condoms before and after intervention was 10.7%,95%,and the difference was statistically significant(?~2=33.384,P<0.05);Before the intervention,the use of condoms was always 3.8%,after the intervention was 83.3%,and the difference was statistically significant(?~2=20.218,P<0.05);Before the intervention,the use of condoms always accounted for 10.4%of commercial sex,and after the intervention was 80.0%.The difference was statistically significant(?~2=14.889,P<0.05);Before the intervention,male homosexual sex always used condoms 6.9%,and after the intervention it was 71.4%.The difference was statistically significant(?~2=14.992,P<0.05).The intervention was effective.Conclusion:1 The HIV/AIDS epidemic in the elderly aged 50 and above in Yixing has been increasing year by year.Sexuality is the main route of transmission for this population.HIV/AIDS cases in the elderly show regional aggregation,and medical institutions are the main channels for the discovery of HIV/AIDS in the elderly.2 The sexual needs of the HIV/AIDS population aged 50 and above in Yixing are widespread.The main factors that affect the risk of HIV infection in the elderly are sexual history,same-sex sex,commercial sex,and non-marital sex.Condoms are not always used.Unstable marital status is also associated with HIV infection.3 To enhance the dissemination of knowledge and behavioral interventions on the prevention of HIV and AIDS infection in the elderly,which can effectively increase the awareness of HIV and AIDS knowledge in the elderly and effectively change the non-marital behavior and condom use habits of HIV/AIDS cases in the elderly,Reduce the second generation of HIV transmission.
