Application Study Of "Caregiver Role Strain" Based On NNN Link In Family Caregivers Of Hospitalized Elderly Stroke Patients

Objective:According to the nursing diagnosis of "caregiver role Strain" in the NNN link(the organic link of nursing diagnosis,nursing outcomes classification and nursing interventions classification),the nursing care plan of the elderly stroke family caregivers in hospital was formulated and applied to the elderly stroke family caregivers in hospital by the nursing outcomes linked by the nursing diagnosis and the nursing interventions linked by the nursing outcomes.Compared with the traditional nursing activities,explore the clinical application effect of "caregiver role Strain" NNN link in Family Caregivers of hospitalized elderly Stroke patients.Method:Family caregivers of 77 elderly stroke patients who met the research compliance with inclusion exclusion criteria were divided into intervention group(38 cases)and control group(39 cases)according to randomized controlled method.The control group was given usual nursing interventions,while the intervention group accepted the "caregiver role Strain" nursing model based on NNN link and usual nursing interventions.According to nursing outcome scale(caregiver emotional health,caregiver lifestyle disruption,caregiver-patient relationship,caregiver performance: direct care and indirect care,caregiver physical health,caregiver well-being,caregiver performance),the outcome of family caregivers were evaluated at before and after expectation,and expectation after the intervention.when elderly patients with stroke in the intervention group before discharge the implementation of nursing interventions for NNN links in the intervention group is described and statistically analyzed.At the same time,The semi-structured interviews were used to interview family caregivers of stroke patients in the intervention group to improve the intervention plan.Result:1.There was no significant difference in the general data of stroke patients and their family caregivers between the intervention group and the control group(p > 0.05),indicating that the baseline data of the two groups were balanced and comparable.2.The variance analysis of eight outcome scores of caregiver emotional health,caregiver lifestyle disruption,caregiver-patient relationship,caregiver performance: direct care and indirect care,caregiver physical health,caregiver well-being,caregiver performance before and after intervention was carried out in the intervention group and the control group.The results showed that:The time effect was statistically significant(P < 0.05),indicating that the scores of the eight outcomes varied with time.Except for caregiver lifestyle disruption outcome,the other seven outcomes scores of interaction between time and treatment had statistical significance(P < 0.05),which indicated that the seven outcomes scores in intervention group and control group had different trends with time.In addition to caregiver lifestyle disruption outcome,the other seven outcomes scores of treatment effect also had statistical significance(P < 0.05),indicating that there were differences in the seven outcomes scores between the two groups,and the intervention group scored higher than the control group.3.Compared with the control group,except for the outcome of caregiver lifestyle disruption,the percentage of the intervention group reached the expected level was higher than that of the control group,and the difference was statistically significant(P < 0.05).4.Statistical analysis was made on the implementation of nursing interventions in the intervention group.The top 10 were:teaching: individual,caregiver support,anticipatory guidance,self-care assistance,health system guidance,coping enhancement,family involvement promotion,anxiety reduction,emotional support and role enhancement.5.Semi-structured interview results: Family caregivers believe that they can benefit from the NNN link care model,which meets their care needs,reduces the burden of care,and mobilizes the initiative of patients and caregivers.Conclusion:Nursing model of "Caregiver Role Strain" based on NNN link provides theoretical and practical basis for clinical nursing intervention.The application of NNN link improves family caregiver's care ability(direct care and indirect care),improves caregiver-patient relationship,strengthens caregiver's role ability,improves caregiver's emotional and physical health,It also improves the health status and social life satisfaction of caregivers,and reduces the burden of care.
