Study On Caregiving Competence Of Stroke Caregivers In Hospital

Object: To explore the caregiving competence of informal caregivers of stroke patients and describe the correlation between caregiving competence and caregiver self-efficacy before discharge.Furthermore,exploring the mediating role of copying style between caregiver self-efficacy and caregiving competence in informal caregivers,in order to nurses and health decision-makers provide scientific reference for developing intervention of informal caregivers before discharge.Methods: The main methods are quantitative research and qualitative research.Taking the convenience sampling method to extract 396 stroke patients and their informal caregivers as the research of the questionnaire.The questionnaires included two parts: stroke patient part included:(1)questionnaire of general demography materials of stroke patients,(2)Activities of daily living(ADL);informal caregiver part included:(1)questionnaire of general demography materials of informal caregivers,(2)Family Caregiver Task Inventory(FCTI),(3)General self-efficacy scale(GSES),(4)simplified coping style questionnaire(SCSQ);Where,SPSS 23.0 for statistical description,t-test,one-way ANOVA analysis,relative,multiple linear regression analysis and hierarchical regression analysis are used.A semistructured interview including 12 cases was used as complementary of quantitative study,in order to provide sufficient description of the phenomenon.Results: 1.The total score of FCTI was 12.65±5.13 which is lower than the norm value.Providing care according to care-receiver's needs is better than other dimension,Balancing caregiving needs and one's own needs to maintain,and dealing with the needs of dependents are difficult.The Linear Regression analysis showed stroke patients' Payment type of treatment,degree of disability,previous caregiving experience,caregiver educational level,caregiving time,caregiver health conditions,caregiver self-efficacy and positive coping style will influence caregiving competence.2.Caregiving competence was negatively correlated with the caregiver self-efficacy(r=-0.542,P<0.01),There were negative correlation between caregiver competence and positive coping style(r=-0.648,P<0.01),and positive correlation between caregivers' self-efficacy and positive coping style(r=0.655,P<0.01).3.Hierarchical regression analysis shows: Positive coping style mediates the relationship between caregiver self-efficacy and caregiver competence.The proportions of the mediating effect was 62.54%.Conclusion: 1.The total score of Family Caregiver Task Inventory belongs low level,the necessary measures should be taken to improve the caregiver competence.2.The influencing factors included by caregiver self-efficacy,stroke patients' degree of disability and so on,some targeted measures must be taken to improve the caregiving competence before discharge.3.Caregiving competence is affected directly by caregiver self-efficacy,and indirectly through positive coping style,that is the partial mediation effect of positive coping style.
