Objective:Based on High resolution magnetic resonance imaging?HR-MRI?of cerebral apoplexy and Essen stroke risk score?ESRS?,a new stroke risk score?NSRS?was constructed to study its value in predicting the recurrence risk of stroke patients.Methods:We retrospectively analyzed clinical and imaging data of 50 patients who were confirmed as symptomatic stroke in the Neurology of the first hospital of Shanxi Medical University from August 2017 to August 2018,the primary group?27 cases?and the relapse group?23 cases?.All the patients had undergone head MRI plain scan and HR-MR scan.We use t-test to compare the clinical data and imaging characteristics of two groups' patients,and Logstic multivariate regression analysis to screen out independent risk factors,and establish a new stroke risk scoring?NSRS?model based on ESRS.Two groups of patients were assessed with ESRS and NSRS scores,and their ROC curves were obtained to evaluate the value of ESRS and NSRS in predicting the risk of recurrence of stroke.Results:There was a significant difference between the primary group and the relapse group among smoking,hypertension,hyperlipidemia,intra-plaque hemorrhage?IPH?,lipid rich necrotic core?LRNC?,lumen area?LA?,wall area?WA?,normalized wall index?NWI?,IPH index,IPC index,LRNC index,and GeometryRf,S?3,3?Contrast,WavEnHLS1?P<0.05?.Smoking,hypertension,hyperlipidemia and IPH are independent risk factors for recurrent stroke in patients with ischemic stroke.The ROC curve showed that the area under curve?AUC?ESRS and NSRS scores were 0.897 and0.961.The sensitivity and specificity of ESRS and NSRS in predicting the recurrence of stroke were 0.783,0.852 and 0.913,0.889,respectively.That is to say,it is more accurate to predict the risk of stroke recurrence by combining clinical and imaging data of patients.Conclusion:Both ESRS and NSRS have better predictive value for the recurrence of stroke,but NSRS model is more detailed for the risk stratification of stroke.The score of NSRS is more accurate,and more valuable for predicting the recurrence of ischemic stroke.
