Objective:1.To understand the cognition and application of dental medical staff and residents in Shanxi Province;2.To investigate the prevalence of caries and the retention of sealants in the first permanent molars of the children's oral disease comprehensive intervention program in Shanxi Province,and analyze the influencing factors;3.Exploring a prevention model suitable for children's oral diseases in our province,and providing a basis for the province to formulate relevant oral health policies and strategies.Methods:1.To investigate the cognition and application of dental clinic personnel and residents in Shanxi Province:using constant volume random sampling method,randomly select 3 counties(districts)in 11 cities in Shanxi Province,and randomly select 20 dental staff aged 22-60,60 residents aged 22-60 in each counties(districts).The self-administered survey method was used to conduct an oral questionnaire survey on the respondents to understand the basic situation of dental staff,the status of pit and fissure sealant work and the cognition of pit and fissure sealant,and to understand the basic situation of the public and the knowledge about pit and fissure sealant.The database was established,the data was entered in the form of double entry,the data was statistically analyzed using IBM SPSS22.0 software,the single factor analysis was used chi-square test to detect the awareness rate of pit and fissure sealant,and the multi-factor analysis was performed by two-class Logistic stepwise regression.2.To investigate the retention of sealants after 1 year and 2 years of pit and fissure sealant implementation in Shanxi Province:using the method of equal proportion random sampling,in the database of the nesting project in Shanxi Province,11 cities in Shanxi Province were drawn in equal proportions.From January to June,7-9 years old children who had been treated with pit and fissure sealant were reviewed for their retention,shedding and caries in the first permanent molars 1 year and 2 years after operation.The database was established and data entry was performed in the form of double entry.Statistical analysis was performed using IBM SPSS 22.0 software.The chi-square test was used to compare and the retention rate of pit and fissure sealant.The spatial heterogeneity analysis was performed by q statistic statistical method.analysis.Results:1.Investigation and study on the cognition and application of pit and fissure sealant(1)Univariate analysis showed that the awareness rate of pit and fissure sealant of oral medical staff was not statistically different between urban and rural areas,male and female,age,type of work,and education level(P>0.05).The difference between different working years was Statistical significance(?~2=16.676,P<0.05),the awareness rate increased with the increase of working years;the awareness rate of residents'pit and fissure sealant was not statistically different between male and female(P>0.05),and there were statistically difference between urban and rural areas(?~2=9.008,P<0.05).The urban was higher than the rural area.And there were statistical differences between different ages and education levels(?~2=43.451,P<0.05;?~2=50.252,P<0.05),The awareness rate of residents in the 22-30 age group is the highest,at 51.4%,and the awareness rate of residents in the 46-60 age group is the lowest,at 32.5%.The awareness rate of residents increases with the education level,the highest awareness rate is available to residents with undergraduate education and above,at51.6%.(2)Logistic regression analysis of the two classifications shows that the urban and rural areas,age,and education level are favorable factors for the residents surveyed to hear about pit and fissure sealant.2.Evaluation of the effect of pit and fissure sealant(1)The complete retention rate of the first permanent molars at 1 year and 2 years after pit and fissure sealant was 81.63%and 75.11%,respectively.The partial retention rates were 13.43%and 14.19%,respectively.The complete shedding rates were 2.88%and 5.30%,respectively.The caries rate was 2.06%and 4.46%,respectively,and the sputum was 0.07 and 0.16,respectively.(2)Univariate analysis showed that there was no significant difference in the rate of sealant retention between urban-rural,gender after 1 year of pit and fissure sealant(P>0.05),2 years after pit and fissure sealant,there is a significant difference in the retention rate of sealants between urban and rural areas,gender(?~2=5.055,P<0.05;?~2=15.079,P<0.05),and males were higher than females,and rural areas were higher than cities;There was a statistically significant difference in the retention rate of different tooth surfaces between 1 and 2 years after(?~2=131.519,96.922,P<0.05).The retention rate of the facet sealant was significantly higher than that of the buccal/ankle surface.At1 and 2 year after surgery,the retention rate of the sealant was no statistical difference between different tooth positions(P>0.05).(3)The results of spatial differentiation analysis showed that the spatial stratification heterogeneity of the retention rate of the sealant at 2 years after the pit and fissure sealant was greater than that of the postoperative 1 year.The retention rate of the pit and fissure sealant increased from north to south after 2 years.The trend of water fluoride content and the proportion of health technicians play a decisive role in the spatial distribution of sealant retention at 1 year and 2 years after surgery.Conclusion:1.Investigation and study on the cognition and application of pit and fissure sealant(1)Pit and fissure sealant has been incorporated into clinical practice as a routine anti-caries measure,and most dental staff have a positive attitude towards the clinical use of pit and fissure sealant.(2)Oral professionals need to regularly learn and update their oral expertise,and strengthen the professional training of low-grade medical staff to improve their awareness of pit and fissure sealant.(3)There is a lack of knowledge about pit and fissure sealant of the public,and the dental staff should publicize the knowledge of pit and fissure sealant and oral health care through various channels to improve the public's awareness,and ensure the prevention of pit and fissure sealant in the early stage of permanent molar eruption,close measures to reduce the probability of dental caries.2.Evaluation of the effect of pit and fissure sealant(1)The effect of free pit and fissure sealant for the first permanent molars of school-age children in Shanxi province is basically guaranteed.As time goes on,the sealant will wear out and fall off,so it is necessary to follow up and review and re-close in time.(2)Gender,urban-rural,and tooth surface have an impact on the retention rate of the sealant,oral medical staffshould strictly follow the standard for pit and fissure sealant to ensure its quality and quantity.(3)It is recommended to optimize the allocation of medical and health resources,and build a dental disease prevention and control system that is led by government departments,dental medical staff,parents,teachers and the whole society to promote the improvement of children's oral health.
