Objective:To analyze the clinical outcomes of catheter ablation for AF with HF(LVEF?50%),and provide more evidences for the treatment of it.Methods:62 patients with persistent or paroxysmal AF,symptomatic HF,and LVEF?50% were assigned to catheter ablation group or medical-therapy groupfrom Second hospital of Shanxi Medical University(2016.09.01-2018.01.01).There were 32 males,30 females,and 32 were traditional medicine patients and 30 patients in the ablation group.The prognoses were the hospitalization for deteriorating heart failure and the maintenance of sinus rhythm during 12 months follow-up.Comparisons of indicators were the differences between groups in left atrial diameter,LVEF,NT-proBNP,NYHA classifications,6-minute walking test changes.Results:After follow-up of 12 months,the maintenance of sinus rhythm occurred in significantly fewer patients in the ablation group than in the medical-therapy group.After a median follow-up of 19.5 months,the numbers of hospitalizations in the ablation group was significantly lower than that in medical-therapy group(P<0.001).After a follow-up of 12 months,6-minute walk test(P<0.001)and NYHA classification(P<0.001)showed that the ablation group was improved compared with the medical-therapy group.There was no difference in the left atrial diameter between the two groups after 3 months.At 6 monthsand 12 months,the left atrial diameter of the ablation group was reduced than the drug group(P < 0.001);the result of LVEF suggested that the ablation group was higher than that the medical-therapy group(P<0.001).NT-proBNP in the medical-therapy group was higher than in the ablation group(P<0.001).Conclusions:Catheter ablation can effectively maintain sinus rhythm in patients with AF with HF,reduce the rate of hospitalization for worsening HF,improve left ventricle function,and HF symptoms.
