Objective:This study aims to investigate and study the relationship between emotion and pain in patients with ankylosing spondylitis from four aspects,including emotive status,pain degree,coping style and mental resilience.On the basis of the previous investigation and research,the SEM model was established by using Amos,M-plus and other tools to explore the influence of emotive state on the pain degree on the basis of pain cognition,as well as the mediating role of mental resilience and coping style in the emotive state and pain degree.In the pain intervention of patients with AS,a comparative study was conducted on the conventional treatment and the combined treatment of conventional treatment and rational emotive therapy,and the results showed the effect of rational emotive therapy on relieving pain of patients with AS through emotional intervention.Methods:Convenient sampling method was adopted in the rheumatology and immunology department of a class a hospital.According to the inclusion criteria,the 200 AS patients to be studied were screened.The male and female ratio of the selected patients should be2.23:1,and most of the patients were under 40 years old.An investigation was conducted on the emotion and pain of 200 AS patients selected and enrolled.The relationship between emotion and pain of AS patients and the mediating role of mental resilience and coping style in the emotional status and pain degree were analyzed using the data of the previous investigation on emotion and pain.Of the 200 AS patients investigated,100 were selected for the intervention controlstudy.Among them,50 patients were in the experimental group,and rational emotive therapy was added on the basis of conventional drug therapy.A control group of 50 patients was treated with conventional drugs alone.After the treatment,psychological measurement and pain assessment were conducted again for the patients in the experimental group and the control group.Independent sample t test and paired sample t test were used to analyze the intervention effect of rational emotive therapy on emotional and inflammatory pain in AS patients.Results:1 The baseline survey results1.1 In the overall evaluation of pain,the overall pain degree score of AS patients was 8.29 1.16,and the night pain degree score was 7.76±1.36,which was at the severe level.In the specific pain measurement,the BASDAI score of AS patients was 7.91±0.83,and the McGill score was 181.59±13.43.1.2 In terms of emotional status,the positive emotional score of AS patients was26.01±4.95,significantly lower than the average score,while the negative emotional score was 36.70±3.64,significantly higher than the average level.In terms of coping style,the positive coping score of AS patients was 14.41±3.15,significantly lower than the average,while the negative coping score was 18.36±2.71,significantly higher than the average.In terms of mental resilience,the total score of mental resilience of AS patients was 61.24 ± 13.65,of which the strength dimension was 22.57 ± 4.93,the resilience dimension was 34.30±4.81,and the optimism dimension was 9.37±2.90.The total score of mental resilience of patients was statistically different from that of community residents in China.1.3 In AS patients,positive emotions are positively correlated with mental resilience and its dimensions,while negative emotions are negatively correlated with mental resilience and its dimensions.Mental resilience and its dimensions were negatively correlated with the degree of pain.Positive emotions were negatively correlated with pain,while negative emotions were positively correlated with pain.Positive emotionswere positively correlated with all dimensions of coping style,while negative emotions were negatively correlated with all dimensions of coping style.The dimensions of coping style were negatively correlated with the degree of pain.1.4 Positive and negative emotions have significant influence on mental resilience and its dimensions.Mental resilience and its dimensions have significant influence on the degree of pain.Positive and negative emotions have significant influence on the degree of pain.Positive and negative emotional state has significant influence on coping style.Coping style has significant influence on the degree of pain.1.5 The deviation correction Bootstrap test showed that the 95%CI of indirect effects was(0.09~0.24,P<0.001),excluding zero,indicating the existence of indirect effects.In other words,psychological resilience and coping style had significant mediating effects between emotional state and pain degree.2 Results of intervention studies2.1 There were significant differences in the four scores of pain degree between the experimental group and the control group before and after intervention and conventional treatment(P<0.01).No significant difference was found in the scores of four items of pain degree between the experimental group and the control group before intervention(P>0.05).After the intervention,there were significant differences between the experimental group and the control group in the overall pain degree,McGill score and BASDAI score(P<0.01),but no significant differences in the night pain degree(P>0.05).2.2 The positive and negative emotions in the experimental group were significantly different before and after psychological intervention and clinical treatment(P<0.01),and the positive emotions score was significantly increased,while the negative emotions score was significantly decreased.In the control group,there was no significant difference in positive emotions before and after psychological intervention and clinical treatment(P>0.05),while there was significant difference in negative emotions(P<0.01),and the negative emotions score was significantly reduced.The results of the intergroup comparison showed that there was no significant difference in positive and negative emotions between the experimental group and the control group before the intervention(P>0.05),and there was a significant difference after the intervention(P<0.05).2.3 Before and after intervention and conventional treatment,there were significant differences in the scores of psychological resilience and its dimensions in the experimental group(P<0.01),and the total scores of psychological resilience and its dimensions were significantly improved.The tenacity score of the control group was significantly different(P<0.01),and the tenacity score was significantly improved.No significant difference was found between the control group and the control group in optimism score,strength score and mental resilience total score after intervention.The intergroup comparison showed that there was no significant difference between the experimental group and the control group in the psychological resilience and the pre-evaluation scores of each dimension(P>0.05),and there was a significant difference in the post-evaluation scores(P<0.05).2.4 The level of coping style in the experimental group was significantly different before and after psychological intervention and conventional treatment(P<0.01),and the positive coping score was significantly increased,while the negative coping score was significantly decreased.In the control group,the negative response level in the post test was significantly different from that in the previous test,and the score of negative response was significantly reduced(P<0.01).No significant difference was found between the two groups(P>0.05).The comparison between the groups showed that there was no significant difference in the coping style level between the experimental group and the control group in the pretest(P>0.05),and there was a significant difference between the posttest and the control group(P<0.05).Conclusion:1.The mediating effect between emotional state and degree of pain was significant in resilience and coping style of AS patients.Positive and negative emotional state can not only directly affect the pain degree of AS patients,but also further change the pain degree through mediating factors such AS psychological resilience and coping style.2.The mediating effect between emotional state and degree of pain was significantin resilience and coping style of AS resilience and coping style.3.The main effects of rational emotion therapy on pain and emotional intervention for AS patients are AS follows: Replace unreasonable belief with reasonable belief on the current situation,and improve the patients' adaptability by improving their stress response;At the same time,the protective factors of mental resilience were activated to improve the level of mental resilience of patients.In addition,by alleviating and eliminating the negative emotional state of patients and replacing it with positive emotional state,the inflammatory pain of AS can be finally alleviated by combining the above approaches.
