Explore On The Current Situation Of Application And Sharing Of Health Examination Big Data

Background and PurposeDisease spectrum has changed gradually,with the different diet structure and environment,as well as the fast pace of life and work in the era of internet and big data.The incidence of chronic diseases is increasing and more of the sick population is younger;With even more people are in sub-health.The public pays more attention to health status and health management.In recent years,the population of physical examination is gradually expanding,and the items of examination are greatly enriched.With the wide application of information-based health record system,more and more digitized health data are accumulated,which makes it to be an important part of the big data in health care.However,the situation on the application and data sharing in this field remain unknown.This study wants to explore a rough description of its current situation,discuss and analyze the value and the potential factors that impede the development of its application and sharing,and provide some suggestions accordingly.Content and MethodLiterature,academic papers and government documents,were searched from professional literature databases,portal websites in English and Chinese,and official websites.Then,he collected data are categorized into five aspects:policy,conference report and related information;practice of application and data sharing;progress at home and abroad;difficulties and potential problems;disputes and analysis.In addition,a small questionnaire was drawn up to department of physical examination in 38 medical institutes as a supplement to literature research.The application fields and its sharing situation&level and trend of data produced in physical examination were analyzed through comparative analysis on the basis of literature summary and questionnaire survey.Some qualitative research method,including PEST analysis,comprehensive analysis and case study,and stakeholder analysis method,were also used to analyze the potential problems and bottlenecks during application and sharing of health examination data.Some suggestions were provided through discussing in theory and thinking in practice,accordingly.Result156 core documents were selected from 884 relevant documents,Literature summary analysis suggests that the big data of health examination has been applied in many aspects.Among them,articles concentrated in application through internet(22),its benefits for promoting self-health management(28),as controls to the diseases in clinical researches(17),and articles of predicting diseases and their developmental trend in public health(19).The valid response rate of the questionnaires was 100%,from 38 medical institutes in 15 provinces and cities.Results showed that Among them,informationalized health examination and record system was 100%in the surveyed institutes;34%of medical institutions had achieved small-scale sharing,and 83%of the institutions had consciously collected health examination data for other purposes,including health management(89.5%),as comparison for clinical research case groups(42.1%),as data support for other medical scientific research in the hospital(55.3%),and cooperation with other institutions to provide data support for medical scientific research(39.5%).Based on all the 80 valid questionnaires,the results show that the most concerns for individuals about health examination data sharing is data security and information disclosure(73%).The most likely reasons for the sharing barrier were the unclear policies,regulations and ethical problems,and 60%of respondents believed that the informed consent of privacy data sharing must be signed.DiscussAt present,the informationalized management of health examination data makes it possible to share between institutes.The health data has not been fully applied and shared;the reasons are two aspects.Common problems faced by data sharing,those are the protection of data security and individual privacy;policy support,legal and ethical guaranteed;acceptance of sharing concepts of public,etc.The questionnaire also reflects the above concerns of institutions and data management centers.Specially,in view of the big data health,the value of health data has not been fully recognized by investigators and directors of the institutes,the department are generally determined as profit-making organizations,thus,lacking corresponding planning of application of the data for research,and lacking data management and mining staff.ConclusionApplication and development of big data derive from health examination has broad prospects.The data is still be mainly used for physical examination business,reports and individual follow-up health management.The other applications,such as integration and reuse,supporting clinical research,etc remains to be promoted and excavated.The recognition and acceptance level of sharing on health data are relatively high in departments of the tertiary medical institutions;however,most of the big data health have not been applied outside the business.The main obstacles to the application and sharing of data of health examination are consistent with the common problems of data sharing in health care,including policy support and data security,legal guarantee and and ethical consensus.While,the economic profit orientation of the departments in medical institutions,and the lack of investigators and data mining personnel,also limit application and potential value of data health.In order to promote the integration of health data,especially with clinical and scientific data,including genome,public awareness of data sharing should be enhanced,special standardization of big data of health examination should be established,based on compliance with all data sharing guidelines.
