Objective:Plasma level of fibroblast growth factor(FGF)21 in preeclampsia patients was determined to explore the correlation between FGF21 level and disease in the study,so as to provide a new theoretical basis for the prevention and treatment of preeclampsia.Methods:80 cases of pregnant women who were regularly prenatal examinated and delivered in the first affiliated hospital of Soochow University from January 2017 to December 2018 were included in the study.49 preeclampsia patients were selected as the preeclampsia group and 31 eases of healthy pregnant women in the same period were selected as the control group.The plasma FGF21 level was determined by enzyme linked immunosorbent assay(ELISA).Clinical data and clinical biochemical test indexes of pregnant women and clinical data of newborns in the two groups were collected.SPSS 21.0 statistical software was used for data analysis.P<0.05 means the difference is statistically significant.Results:1.Comparison of general information of pregnant women:Blood pressure of preeclampsia group were significantly higher than those of the control group(P<0.05).The family history of hypertension in the preeclampsia group was significantly higher than that in the control group(P>0.05).There were no statistical differences between the preeclampsia group and the control group in age,pre-pregnancy BMI,and ratio of multiparous women(P>0.05).2.Comparison of maternal and infant outcomes:The preeclampsia group had a significantly less delivery gestational weeks than the control group,and the cesarean section rate was significantly higher than that of the control group(P<0.05).Delivery gestational weeks of the early-onset/severe preeclampsia group were less than the late-onset/mild group(P<0.05).Among the complications of preeclampsia pregnancy,preterm delivery,fetal intrauterine growth restriction(FGR)and pleural and peritoneal effusion had the highest incidence rate.Preterm delivery and FGR have statistical differences between groups by onset time and severity(P<0.05).The neonatal weight of the preeclampsia group was signifieantly lower than that of the control group,and the neonatal weight of the early-onset/severe group was significantly lower than that of the late-onset/mild group(P<0.05).Apgar score at 1 min and 5 min after birth and gender of newborn had no significant difference between the preeclampsia and control group or between groups by onset time and severity of preeclampsia(P>0.05).3.Comparison of FGF21 level and biochemical test indexes:Compared with the control group,preeclampsia group had significantly elevated levels of FGF21,alanine aminotransferase(ALT),aspertate aminotransferase(AST),blood urea nitrogen(BUN),serum creatinine(Cr-S),blood uric acid(UA),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and 24-h urea protein(P<0.05).The levels of FGF21,ALT,BUN,Cr-S,UA,TG and 24-h urea protein in the early-onset group were significantly higher than those in the late-onset group(P<0.05).Compared with the mild group,the levels of FGF21,ALT,BUN,Cr-S,UA,TC,TG and 24-h urea protein in the severe group were signifieantly increased(P<0.05).4.Correlation analysis and regression analysisAfter Pearson or Spearman analysis,the plasma FGF21 level of 80 pregnant women was positively correlated with systolic blood pressure,diastolic blood pressure,mean arterial pressure,BUN,UA,TC,TG and LDL-C,which negatively correlated with the delivery gestational weeks and neonatal weight.In 49 cases of preeclampsia,plasma FGF21 level was positively correlated with mean arterial pressure,diastolic blood pressure,TC and TG.Binary Logistic regression analysis found that FGF21,UA and LDL-C were risk factors for preeclampsia.5.Diagnostic efficacy analysis of FGF21 in preeclampsiaROC curve was used to analyze the sensitivity and specificity of plasma FGF21 level for the diagnosis of preeclampsia.The area under the curve(AUC)of FGF21 level for the diagnosis of preeclampsia,early-onset preeclampsia,severe preeclampsia and preeclampsia with complications were 0.670,0.717,0.735 and 0.711 respectively.AUC of FGF21 level for the diagnosis of severe preeclampsia were the biggest.Conclusion:1.In this study,the plasma FGF21 level of preeclampsia patients was significantly higher than that of normal pregnant women,especially in patients with early-onset/severe preeclampsia.Elevated levels of FGF21,UA and LDL-C were risk factors for preeclampsia.3.FGF21 level was positively correlated with mean arterial pressure,diastolic blood pressure,TC and TG in preeclampsia patients.Plasma FGF21 level could be auxiliary diagnosis of preeclampsia,especially for the diagnosis of severe preeclampsia.
