Objective:To explore the correlation between blood lipid and renal function in severe preeclampsia pregnant women complicated with hypothyroidism.Methods:One hundred and twenty pregnant women with severe preeclampsia who were treated with in the first affiliated Hospital of Guangxi Medical University(Hereinafter referred to as "our hospital")in September 2018 were divided into hypothyroidism group(37 cases)and simple severe preeclampsia group(83 cases).All pregnant women had no other complications.120 normal late pregnant women who were hospitalized and delivered in our hospital at the same time were selected as the control group.All pregnant women had no other complications.Blood samples were taken through elbow vein after 12 hours of fasting.Serum thyrotropin,free triiodothyronine,free thyroxine,serum TG,total cholesterol,low density lipoprotein cholesterol,high density lipoprotein cholesterol,apolipoproteins B,apolipoproteins A1,lipoprotein(a)and serum creatinine,uric acid,cystine C and other related indexes were measured.The related examination indexes of the three groupswere compared and analyzed.Results:(1)The results showed that the levels of blood lipids(TC,TG,LDL-C,apoA1,apoB,Lp(a)in simple severe preeclampsia group and severe preeclampsia complicated with hypothyroidism group were significantly higher than those in control group(P < 0.05).The levels of blood lipid(TC,TG,LDL-C,apoB,Lp(a)in severe preeclampsia complicated with hypothyroidism group were significantly higher than those in simple severe preeclampsia group(P <0.05).(2)The levels of Scr,UA and CysC in simple severe preeclampsia group and severe preeclampsia complicated with hypothyroidism group were significantly higher than those in control group(P < 0.05).The level of Scr in severe preeclampsia complicated with hypothyroidism group was higher than that in simple severe preeclampsia group,and the difference was statistically significant(P < 0.05).There was no significant difference in the levels of UA and CysC between the two groups(P > 0.05).(3)Correlation analysis: there was a positive correlation between TSH level and serum TG,TC,LDL-C,apoB,Lp(a),Scr,Cysc C levels in severe preeclampsia(P < 0.05).(4)The incidence of low birth weight and premature delivery in severe preeclampsia complicated with hypothyroidism group was significantly higher than that in simple severe preeclampsia group,and the difference was statistically significant(P <0.05).There was no significant difference in the mode of termination of pregnancy between severe preeclampsia complicated with hypothyroidism group and simple severe preeclampsia group(P > 0.05).Conclusion:(1)Severe preeclampsia is closely related to hypothyroidism and dyslipidemia.Compared with normal pregnant women and patients with severe preeclampsia,the level of TC?TG?LDL-C?apoB?Lp(a)in patients with severe preeclampsia with hypothyroidism is higher.(2)Patients with severepreeclampsia complicated with hypothyroidism have a severe renal burden.Cystatin C is affected by both severe preeclampsia and hypothyroidism when monitoring renal function,which should be evaluated combined with other indexes(such as creatinine and uric acid).(3)Patients with severe preeclampsia with hypothyroidism have a high risk of developing premature infants and low birth weight infants.
