Objective To explore the correlation between prognosis and its influencing factors in patients with prostate cancer Using Prostate Cancer Database.To further improve the information management database of prostate cancer patients.To make clear the important role of prostate cancer database,in assessing tumor progression and prognosis,and rationally guiding clinical treatment.Methods Retrospectively collected clinical case data and basic information needed for research of patients with prostate cancer from January 2012 to December 2018 in our department of urology.Patient case data is entered into the database according to the fields established in the prostate cancer database.Statistical methods were used to compare age,tPSA,PSAD,BMI,fasting blood glucose,blood cholesterol,blood triglycerides,tumor stage,and Gleason scores in patients with prostate cancer.And the relationship between biochemical recurrence and age,tPSA,PSAD,BMI,fasting blood glucose,blood cholesterol,blood triglyceride,postoperative pathological Gleason score and tumor stage after radical prostatectomy.Results Basically complete the establishment of information management database for prostate cancer patients.Through the analysis of clinical and follow-up information of prostate cancer patients in our hospital for nearly 7 years:(1)The number of patients with prostate cancer in the first year of urology in our hospital has an increasing trend.(2)Gleason score<7 and Gleason score>8 The age-average comparison between the two groups showed that the prostate cancer patients with higher Gleason scores were older,but not statistically significant;the patients in the biochemical recurrence group were younger than those without biochemical recurrence(P<0.05).(3)The mean value of tPSA and PSAD in patients with prostate cancer with Gleason score>8 was higher than that in patients with Gleason score<7.Among patients with prostate cancer who had biochemical recurrence within 2 years after RP,tPSA and PSAD were significantly higher than those without biochemical recurrence(P<0.05).(4)Patients with Gleason score>8 had lower BMI than patients with Gleason score<7(P<0.05);fasting blood glucose,blood cholesterol and triglyceride levels were not statistically correlated with Gleason score and biochemical recurrence.(5)Patients with Gleason score>8 had higher tumor stage than patients with Gleason score<7;patients with Gleason score>8 had higher lymph node metastasis than patients with Gleason score<7;patients with Gleason score>8 had significantly higher bone metastasis than Gleason score<7 patients;biochemical recurrence group patients with tumor stage higher than no recurrence group.(6)The Gleason score in patients with biochemical recurrence was higher than that in patients without biochemical recurrence.Conclusion The number of patients with first-stage prostate cancer in our department of urology from 2012 to 2018 has been increasing.There was no significant correlation between the age of patients with prostate cancer and the Gleason score of puncture pathology.Patients with younger prostate cancer have a higher risk of recurrence after RP,and have a worse prognosis than older patients.There was a positive correlation between tPSA and PSAD in prostate cancer patients and Gleason score in puncture pathology.Both were also positively correlated with the rate of biochemical recurrence after RP.In this study,patients with low BMI had higher Gleason scores in patients with prostate cancer.Fasting blood glucose,blood cholesterol and triglyceride levels have no predictive effect on prostate cancer prognosis.There is a positive correlation between prostate cancer staging and Gleason score.Patients with Gleason score>8 had significantly higher tumor stage T,and were more prone to lymph node metastasis and bone metastasis.The rate of recurrence of RP after prostate cancer in patients with T≥3 was significantly increased.The information management database of prostate cancer patients improves the convenience of patient information data analysis and plays an important role in this study. |
