Objcctive:to explore the related factors of recurrence in patients with epithelial ovarian cancer after clinical complete remission,analyze the influence ratio of each factor,and provide guidance for clinical diagnosis and treatmentMethods:A retrospective study was made of 55 patients with epithelial ovarian cancer who were admitted to the First Affiliated Hospital of Soochow University from January 2013 to December 2015 and who achieved clinical complete remission after cytoreductive surgery and chemotherapy.Operative status,pathological information and prognosis of the patients were collected.Age,body mass index,FIGO stage,histological type,histological grade,maximum residual diameter,lymph node metastasis,ehemotherapy cycles,cancer antigen 125(CA125)level,estrogen receptor(ER),Progesterone receptor(PR),Systemic immune inflammation index(S?),Platelet-to-lymphocyte ratio(PLR),Neutrophil-to-lymphocyte ratio(NLR),Monocyte-to-lymphocyte ratio(MLR),systemic immune inflammatory response index(S?)were analyzed.The optimal cut-off values of NLR,PLR,MLR and S? were calculated by using the receiver operating characteristic curve(ROC)curve.Kaplan-Meier single factor survival analysis,Cox regression single factor and multivariate survival analysis were used to determine the influencing factors and independent risk factors of recurrence in patients with epithelial ovarian cancer.Results:In this study,a total of 71 patients with epithelial ovarian cancer were admitted to our hospital from January 2013 to December 2015,including 10 cases of failure to achieve clinieal remission after surgery and chemotherapy,and 6 cases of loss of follow-up.55 cases were finally included in this study.1.The results of ROC curve showed that the best cutoff values of preoperative PLR,NLR,MLR,S? and Ki67 indicators for recurrence of patients with epithelial ovarian cancer were 108.04,2.69,0.26,720.79 and 52.5%.2.The results of Kaplan-Meier single factor survival analysis showed that FIGO stage,histological type,histological grade,maximum residual diameter,lymph node metastasis,cycles of chemotherapy,PLR level,and Ki67 were associated with recurrence of patients with epithelial ovarian cancer(P<0.05).3.The results of Cox multivariate analysis showed that advanced FIGO stage(HR=3.362,95%CI:1.403-8.058),postoperative residual lesion diameter?1cm(HR=2.109,95%CI:1.250-3.556)were Independent risk factors for recurrence of epithelial ovarian cancer(P<0.05).Conclusions:1.Factors influencing recurrence of epithelial ovarian cancer patients with clinical complete remission include advanced(III/IV)stage epithelial ovarian cancer,histological type,postoperative residual lesion diameter,lymph node metastasis,chemotherapy frequency,Ki67 index and PLR level.2.Independent risk factors for recurrence of epithelial ovarian cancer patients with clinical complete remission include advanced FIGO stage and postoperative residual lesion diameter?1cm.3.Preoperative PLR level has a higher predictive value for recurrence of epithelial ovarian cancer than NLR level,MLR level and SII level.
