Purpose:To investigate the independent factors influencing the onset and gas absorption of neonatal pneumothorax.Methods:In the first part of this study,we retrospectively reviewed the clinical data of neonates,admitted to Children's Hospital of Soochow University between January 1,2015,and December 31,2018.Neonates with pneumothorax were included in the pneumothorax group(n=200).Neonates without pneumothorax,in the same admission period,were randomly selected as the control group(n=600).We performed both univariate and multivariate logistic regression analysis to identify predictive factors for neonatal pneumothorax.In the second part,the pneumothorax group was divided into two subgroups,?7 days(n=158)group and>7 days(n=25)group,according to the gas absorption time.Univariate and multivariate logistic regression analysis were used to analyze the risk factors affecting gas absorption time of pneumothorax.Results:We found five significant predictors of neonatal pneumothorax,including Cesarean section(OR 2.16,95%CI 1.42-3.30,P=0.0003),meconium aspiration syndrome(OR 38.44,95%CI 9.97-148.28,P<0.0001),respiratory distress syndrome(OR 2.78,95%CI 1.27-6.09,P=0.0107),pneumonia(OR 29,25,95%CI 13.24-64.60,P0.0001)and mechanical ventilation(OR 3.73,95%CI 2.33-5.99,P<0.0001).In addition,conservative treatment(OR 41.32,95%CI 8.05-211.96,P<0.0001)and mechanical ventilation time after pneumothorax(OR 1.81,95%CI 1.36-2.39,P<0.0001)were signifcantly associated with gas absorption time.Conclusion:Cesarean section,meconium aspiration syndrome,respiratory distress syndrome,pneumonia and mechanical ventilation were predictors of neonatal pneumothorax.After the occurrence of neonatal pneumothorax,conservative observation and mechanical ventilation time were risk factors affecting gas absorption time.
