Leukoaraiosis(LA)not only causes clinical symptoms such as cognitive dysfunction and dementia,but also psychological diseases such as depression.Confirmed in a previous study of type 2 diabetes mellitus(T2DM)accelerate the process of the leukoaraiosis,and often co-exist with depression.What is the risk of depression in patients with type 2 diabetes with leukoosteoporosis? but there is little research on this respect.This study explored the incidence of depression in patients with leukoaraiosis with type 2 diabetes mellitus and related risk factors for depression.1.Collected from January 2017 to October 2018 in departments of neurology of leukoaraiosis of 130 patients with type 2 diabetes as a study group,select the same period in the hospital without diabetes patients with leukoaraiosis 70 cases as control group,there was no statistically significant difference in gender,age,culture level,high blood pressure,coronary heart disease,the degree of leukoaraiosis,aspects such as smoking and drinking between two groups.2.The patients were made diagnosis of depression according to the Diaguostic and Statistical Manual of mental disorders-fifth edition(DSM-V).The incidence of depression was compared between the study group and the control group.3.Demographic data,vascular risk factors,biochemical indexes and imaging data of the patients in the study group were collected,and the Montreal cognitive scale score(MoCA),Hamilton anxiety scale(HAMA)and Pittsburgh sleep quality index score(PSQI)of the patients in the study group were calculated.4.According to DSM-V,130 patients were divided into depression group and non-depression group,and the severity of depression was evaluated according to the HAMD.Univariate analysis was performed on the risk factors of depression in the study group,and the variables with statistical significance were included in the Logistic analysis model to further analyze the independent risk factors of depression in patients with leukoaraiosis combined with type 2 diabetes.Results of this study: 1.The incidence of depression in the study group was 46.92%(61 cases),and the incidence of depression in the control group was 30.00%(21 cases).The difference between the two groups was statistically significant(P<0.05).2.The incidence of non-depression was 53.08%(69 cases),the incidence of depression was 46.92%(61 cases),the incidence of mild depression was 27.69%(36 cases),the incidence of moderate depression was 16.15%(21 cases),and the incidence of severe depression was 3.08%(4 cases).3.HbA1c(7.84±1.77)vs(6.89±1.56)and homocysteine(Hcy)16.50(9.05,19.75)vs 12.30(8.90,17.40)levels in patients with leukoaraiosis and type 2 diabetes had statistically significant differences between the depressed group and the non-depressed group(P < 0.05).4.The leukoaraiosis score was 14.00(7.00,21.00)vs 8.00(5.00,11.50),and the number of lacunar infarction(LI)(6.90±3.76)vs(5.67±3.14).The difference between the depressed group and the non-depressed group was statistically significant(P < 0.05).5.MoCA score20.00(16.00±23.50)vs22.00(19.00,26.00)and PSQI score(9.70±3.84)vs(6.87±2.78)were significantly different between the depressed group and the non-depressed group(P < 0.05).6.Multivariate Logistic regression analysis showed that HbA1c(OR=1.315,95CI: 1.022 ~ 1.691,P=0.033),LA score(OR=1.118,95CI: 1.041 ~ 1.201,P=0.002),MoCA score(OR=0.877,95CI: 0.785~0.980,P=0.020),PSQI score(OR=1.161,95CI: 1.011 ~ 1.333,P=0.034)is an independent risk factor for depression in patients with leukoaraiosis combined with type 2 diabetes.Conclusion of this study:1.The incidence of depression is higher in patients with leukoaraiosis and type 2 diabetes mellitus.2.Glycosylated hemoglobin,homocysteine,the degree leukoaraiosis,the number of lacunar cerebral infarction,the level of cognitive function and sleep disorders are related to the occurrence of depression in patients with leukoaraiosis combined with type 2 diabetes.3.The degree of leukoaraiosis in patients,glycosylated hemoglobin,sleep disorder,and the level of cognitive function are independent risk factors affecting the degree of depression in patients with leukoaraiosis combined with type 2 diabetes mellitus.
