A Comparative Study Of Two Different Types Of Intraocular Lens Implantation On Visual Quality After Cataract Surgery

Objective:This paper aims to study the visual quality and visual related life quality after phacoemulsification combined with implantation of the difractive aspherical bifocal intraocular lens(BIOL)ZEISS AT LISE 809M and sonofocal intraocular lens(SIOL)ZEISS CTA603P.Methods:This is a retrospective non-randomized study 110 patients(176 eyes)from hospital underwent phacoemulsification combined with intraocular lens from October 2016 to December 2017.56 cases(82 eyes)implant with ZEISS AT LISE 809M and 54 cases(94 eyes)with ZEISS CTA 603P.Three months after the surgeries,the main evaluation indiceswere successively collected,which included the uncorrected distant visual acuity(UDVA,5m),the corrected distant visual acuity(CDVA),the distant corrected interm-ediate visual acuity(DCNVA,33cm),the optical quality analysis system(OQAS)of the MTF cutoff,the Strehl ratio,the OSI,the 100%VA,the 20%VA and the 9%VA.as well as the visual function related quality of life scale(NEI-VFQ-25).Comparative analysis regarding 11 sub-items were implemented within the NEI-VFQ-25 questionnaire.All the data were processed by the SPSS22.0,and conclusions of statistically significant were obtained by p<0.05.Results:1)VA(LogMAR)1.1 In the 809M group,the CDVA was 0.49±0.23and 0.04±0.05 before and after the operation,while the DCNVA was 0.83±0.20 and 0.19±0.12 before and after the operation.Both evaluation criteria showed VA improvement judging by statistically significant of(t=-17.16,p<0.01)and(t=-26.12,p<0.01 Respectively.1.2 In the 603Pgroup,the CDVA was 0.69±0.35 and 0.03±0.05 before and after the operation,while the DCNVA was 1.03±0.35 and 0.62±0.11 before and after the operation.Still,both evaluation criteria showed VA improvement judging by statistically significant of(t=-18.31,p<0.01)and(t=-11.26,p<0.01)respectively.1.3 Between the two groups,there was no significant difference in the CDVA(t=1.56,p>0.05).However,there existed significant difference in the near vision(t=-24.79,p<0.01).2)Analysis of objective visual quality2.1 In the 809M group,the MTF cutoff value was 13.22±7.31 and 28.45±10.50before and after the operation,while the OSI was 4.60±2.66 and 2.71±1.57before and after the operation,while the average Streh I ratio was 0.11±0.10 and 0.18±0.13 before and after the operation.All three evaluation indices achieved improvement results judging by statistically significant of(t=14.76,p<0.05),(t=-7.67,p<0.05)and(t=9.06,p<0.05),separately.2.2 In the 603P group,the MTF cutoff value was 10.10±4.80 and25.90±9.00beforeand after the operation,while the OSIwas 6.38±2.35 and 2.25±0.69before and after the operation,while the average Streh I ratio was 0.70±0.03 and 0.15±0.04 before and after the operation.Again,all three evaluation indices achieved improvement results judging by statistically significant of(t=15.10,p<0.05),(t=-17.26,p<0.05)and(t=17.26,p<0.05),separately.2.3 Between the two groups,there was no significant difference inthe MTF cutoff value and the mean Streh I ratio(p=0.09,0.11,p>0.05),however,there was significant difference in the mean OSI value(p=0.02,p<0.05).2.4 Regarding the visual acuity of simulated contrast sensitivity between the two groups.Both the 100%VA showed no significant difference(p=0.67,p>0.05)before and after the operation.Bycontrast,the 20%VA and the 9%VA expressed significant difference(p=0.004,0.003;p<0.05).3)Defocus curvesThere were two peaks in the 809M group.While only one single peak was found in the 603P group.In specific,the DCNVA of the 809M group at-3.00D behaved obvioussuperior than that of the 603P group,implying a better performance regarding the near vision.4)NEI-VFQ-25 questionnaireThere were no significant differences in overall health,general vision,eye pain,distant vision,peripheral vision,social function,color vision,dependence and mental health between the two groups(p = 0.88,0.08,0.60,0.36,0.80,0.18,0.41,0.07,0.95;p>0.05).But there were significant difference in near vision and social role limitation between the two groups(p=0.003,0.006;p<0.05).Further,there were no significant differences in age and gender(p=0.16,0.21;p>0.05).Last,there was no significant difference in the total score(p=0.474;p>0.05).Conclusions:1.Both the diffractive bifocal aspheric(ZEISS AT LISE 809M)intraocular lenses and the sonofocal aspheric(ZEISS CTA 603P)intraocular lenses can improve the optical quality of patients after the surgeries.The former one can provide better near vision,but the scattering index was higher than the latter one,which may cause some visual interference.2.Although there was no significant difference in the overall scores of visual function-related life quality after the surgeries,the ZEISS AT LISE 809M intraocular lens was superior to the ZEISS CTA 603P intraocular lens in short range activities.
